Role of surgery in the management of anaplastic thyroid carcinoma: Korean nationwide multicenter study of 329 patients with anaplastic thyroid carcinoma, 2000 to 2012

Abstract: Although ATC is a lethal neoplasm, long-term survival may be acquired in cases in which the aggressive management, including curative resection or RT/concurrent CRT, is possible for therapeutic intent. © 2016 Wiley Periodicals, Inc. Head Neck 39: 133-139, 2017.

“…In this study, surgery with curative intent -therefore excluding debulking procedures -was associated with better survival in stage IVA and IVB but not in IVC patients (Baek et al 2016). Another recent study from Korea shows surgical intervention for selected patients might indeed result in a prolonged survival benefit.…”

“…Further regarding metastatic diseases, from our literature revision emerged only retrospective data of 779 patients [ 10 , 11 , 12 , 22 , 23 , 24 , 29 , 30 , 31 , 32 , 33 , 51 , 52 , 53 ], treated with a multimodality approach ( Table 1 ) and with a median overall survival that ranges from 3 to 28 months.…”

“…Evaluating the survival outcomes according to the different clinical features and treatments, they identified age ≥70 years old, the presence of initial clinical symptoms, distant metastasis and treatment modality as significant risk factors ( p < 0.05). Overall, the patients who underwent curative surgery and adjuvant radiotherapy or concurrent chemoradiation obtained the best survival ( p < 0.05) [ 33 ].…”

“…Radiotherapy. Optimal outcomes in terms of survival and local disease control in ATC require complete or near-complete [no residual tumour (R0) or microscopic residual tumour (R1)] resection followed by high-dose EBRT, with or without concomitant ChT [V, A] [122,123]. However, this multimodal approach can strongly impact QoL and should be reserved for carefully selected patients to ensure clinical benefit.…”
