Role of sonography in the diagnosis of gallbladder perforation

Abstract: Sonography is useful for diagnosing gallbladder perforation and detecting the defect in the gallbladder wall. We believe that sonography should be the first-line imaging modality for evaluating the patients in these cases.

“…Though ultrasound could not specifically identify type I perforations, it was helpful in determining the need for surgical intervention, as it could identify the presence of free fluid, the appearance of which, on guided tap, left no doubts about the pathology. The results of ultrasound in our patients are similar to those quoted in various studies comparing efficacy of ultrasound in detecting gall bladder perforation [13][14][15]. Ultrasonography (USG) and CECT are the most important tools in diagnosing this rare complication.…”

“…Sood et al noted that the sonographic hole sign, in which the defect in GB wall is visualized, is the only reliable sign of gallbladder perforation. 9 The best form of management is early surgery as delay in surgery increases both mortality and morbidity of patient. 10 CONCLUSION: Although GB perforation due to stones is predominantly found in the elderly, it can occur in the young, so high index of suspicion in acute upper abdomen is required.…”

“…Distention of the gallbladder and edema of its wall may be the earliest detectable signs of imminent perforation. The hole sign [10] (a defect in the gallbladder wall) is the most specific finding. Solva et al [11] showed in their study that distention of gallbladder and edema of its wall may be earliest sign of impending perforation on ultrasonography.…”
