Role of postoperative radiotherapy in reducing ipsilateral recurrence in DCIS: an observational study of 1048 cases

Abstract: BackgroundThe objective of the present study was to evaluate the effectiveness of postoperative radiotherapy after breast conserving surgery (BCS) in DCIS in a large patient population treated in clinical practice.MethodsData were provided by the population-based Munich Cancer Registry. Between 1998 and 2014, 1048 female patients with diagnosis of DCIS and treated at two Breast Care Centres were included in this observational study. The effectiveness of postoperative radiotherapy and variables predicting the u… Show more

“…Despite these results, the use of RT after BCS varies considerably between countries. For instance, a study in Japan showed only 41% of RT use after BCS, whereas other series in the UK, 10 Queensland, 32 and Munich 30 showed 62%, 62%, and 66% rates, respectively, with similar rates in the ICSN multicenter study. 25 A study conducted in the Navarra region of Spain reported a 100% RT rate.…”

“…Table 5 shows the results of randomized trials and several large retrospective studies confirming the place of RT in reduction of LR rates after BCS. 5,6,15,[28][29][30] The meta-analysis showed that approximately 50% of LRs were invasive. 31 The benefit of RT increases with time, as the overall absolute LR reduction was 10.5% at 5 years and 15.2% at 10 years (from 28.1% to 12.9%; log rank P < .0001).…”

“…Although previous studies provided evidence for a possible survival benefit in intermediate-risk patients [41], PMRT was not uniformly recommended at that time. The standard RT regimen at the Department of Radiation Oncology of the LMU Munich during the observation period was whole-breast irradiation following BCS (50.4 Gy in 28 fractions) using a 3-dimensional conformal tangential field technique with a photon or electron boost of 10–16 Gy to the tumor bed [42].…”

“…Adding radiotherapy to breast-conserving treatment reduces local recurrence rates but does not influence overall survival or breast-cancer-specific survival. 27,45,46 The added value of conducting a sentinel node biopsy procedure is uncertain. In general, such a procedure is done with mastectomy for DCIS (since there is no opportunity to perform a subsequent sentinel node biopsy) or where there is a high suspicion for invasive disease even where DCIS alone is present in the preoperative biopsy.…”
