Background: In recent years, chronic kidney disease (CKD) has become a worldwide public health issue. Objective: The main aim of this study was to explore the ability of oral activated charcoal to adsorb uremic toxins limiting the progression of CKD and delaying the need for hemodialysis in patients with CKD stages III and IV and to compare its effect with the effect of dry seeds (lentils) as absorbents of uremic toxins. Patients and methods: A randomized controlled clinical trial was conducted on 90 patients with CKD stages III and IV. The patients were divided into 3 groups as following; Group (A): 30 patients received oral activated charcoal in a dose of 3 gm/day, Group (B): 30 cases received dry seeds (raw lentils) in a dose of 30 gm/day, and Group (C): 30 cases as control group on usual conservative treatment. Results: Group A showed that there was significant decrease in serum urea, creatinine, phosphorus and potassium throughout follow up period (12 weeks) compared to Group B and Group C (control group). Group B showed there was significant decrease in serum urea and creatinine throughout follow up period (12 weeks), while there was no significant difference in serum phosphorus and potassium, compared to Group A and Group C. Conclusion: The use of oral activated charcoal and uncooked lentils could be of value in lowering uremic toxins in patients with CKD stages III and IV. Future studies are warranted to confirm such results.

The nephroprotective effect of standardized aqueous root extract of Vetiveria zizanioides (L.) Nash (Family: Poaceae) was investigated in doxorubicin‐induced (20 mg/kg, ip) experimental nephrotoxicity model of Wistar rats. The freeze‐dried aqueous refluxed (4 hr) root extract of V. zizanioides (25, 50; equivalent human therapeutic dose and 100 mg/kg) was administered separately to nephrotoxic Wistar rats (n = 6/group). Supplement of V. zizanioides resulted a dose‐dependent reduction in raised serum creatinine, β2‐microglobulin, and blood urea nitrogen and a subsequent increase in serum total protein and albumin in nephrotoxic rats (p < .05). An attenuation of the doxorubicin‐induced features of renal parenchymal injury was observed on H‐ and E‐stained sections of the kidney tissues. Nootkatone, dehydroaromadendrene, isokhusenic acid, α‐vetivone, and isolongifolene were identified in the methanol extract of V. zizanioides based on the GC‐MS chromatogram analysis. The findings revealed that the supplement of standardized aqueous root extract of V. zizanioides had a significant dose‐dependent nephroprotective activity against doxorubicin‐induced experimental nephrotoxicity. Practical applications Vetiveria zizanioides is a medicinal plant with a variety of therapeutic applications in kidney‐related diseases. Apparently, it is used as a food ingredient due to its fresh and elegant scent and potential bioactivities. The aqueous root extract of V. zizanioides exerted relatively high antioxidant potential in vitro, substantiating the health effects of the plant pertaining to kidney diseases as a potential source of dietary antioxidant. The administration of the plant extract resulted in significant nephroprotection against doxorubicin‐induced experimental nephrotoxicity revealing the significance of V. zizanioides as a promising dietary supplement in the management of kidney disease.

The present research investigates the tuber proteome of the ‘medicinal’ plant Jerusalem artichoke (abbreviated as JA) (Helianthus tuberosus L.) using a high-throughput proteomics technique. Although JA has been historically known to the Native Americans, it was introduced to Europe in the late 19th century and later spread to Japan (referred to as ‘kiku-imo’) as a folk remedy for diabetes. Genboku Takahashi research group has been working on the cultivation and utilization of kiku-imo tuber as a traditional/alternative medicine in daily life and researched on the lowering of blood sugar level, HbA1c, etc., in human subjects (unpublished data). Understanding the protein components of the tuber may shed light on its healing properties, especially related to diabetes. Using three commercially processed JA tuber products (dried powder and dried chips) we performed total protein extraction on the powdered samples using a label-free quantitate proteomic approach (mass spectrometry) and catalogued for the first time a comprehensive protein list for the JA tuber. A total of 2967 protein groups were identified, statistically analyzed, and further categorized into different protein classes using bioinformatics techniques. We discussed the association of these proteins to health and disease regulatory metabolism. Data are available via ProteomeXchange with identifier PXD030744.
