Role of biliary tract cytology in the evaluation of extrahepatic cholestatic jaundice

Abstract: Background:Endoscopic evaluation is critical in assessing the cause of obstructive jaundice. Cytological techniques including bile aspiration and biliary brushings have become the initial diagnostic modality.Aim:The aim of this study is to evaluate the role of endoscopic biliary tract cytology as a diagnostic tool in the evaluation of extrahepatic cholestatic jaundice.Materials and Methods:A total of 56 biliary tract specimens including 34 bile aspirations and 22 biliary brushings from 41 consecutive patients … Show more

“…[1][2][3][4] For that reason, many studies have attempted to identify methods to distinguish benign and malignant cells. [3][4][5][6][7][8] The bile cytological criteria (BCC) proposed by the Japanese Society of Clinical Cytology working group study [9][10][11] are divided into groups A and B. A is classified as cluster criteria ((1).…”

“…CCA can be clinically classified as either “intrahepatic” (IH-CCA) or “extrahepatic” (EH-CCA), and each type requires its own unique treatment plan; in the former, cancer cells originate from the bile duct within the liver, and 10–20% of primary liver tumors form this way 46 . More than half of all CCA-positive patients have EH-CCA, the most common type of CCA 3, 7 .

“…For example, standard cytological tests performed with bile tract brushings only diagnosed CCA in 50% of CCA-positive samples 10 , 13 . Combining cytological with histological approaches, however, may increase the detection rate to 70% 7 , 14 . Furthermore, fluorescence in situ hybridization assay was also combined with biliary brushing specimens for biliary cancer analysis, with 91% of specificity 15 .…”
