Role of Allopurinol in Optimizing Thiopurine Therapy in Patients with Autoimmune Hepatitis: A Review

Abstract: Autoimmune hepatitis (AIH) is a chronic immune mediated liver disease characterized by elevated transaminases, hyper gammaglobulinemia, presence of autoantibodies and interface hepatitis in the absence of a known etiology of liver disease. Thiopurines (azathioprine [AZA]/6-mercaptopurine [6MP]) and steroids remain the first line of treatment of AIH in both children and adults. However, a small proportion of AIH patients are either non-responders or develop side effects with AZA. The metabolism of AZA is comple… Show more

“…It has been demonstrated that mycophenolate mofetil (MMF) ( 5 , 44 51 ), methotrexate ( 52 ), 6-mercaptopurine ( 53 ), allopurinol ( 54 , 55 ), tacrolimus ( 8 , 45 , 56 59 ) and everolimus ( 60 ) are effective and well-tolerated in patients with AIH. Torisu et al ( 61 ) indicated that UDCA monotherapy may be considered for treatment of patients with AIH with a serum ALT level of <200 IU/l.…”

“…Interestingly, modulation of AZA metabolism to reduce side effects through co-administration of allopurinol was suggested and reported in small case series [68][69][70][71][72]. As side effects by this co-medication may be rather mild, this combinatory therapy should be further evaluated in RCTs.…”

“…These findings were included in a subsequent review of the reported experiences with allopurinol in 13 patients with autoimmune hepatitis, and they strengthened the observations in the earlier report (Table 3). 212 The principal side effect was a reversible neuropathy that developed in two individuals (15%) 212,219 …”
