Objective: to analyze the evolution of self-care in hospitalized patients with decompensated heart failure, between the first return after hospital discharge (T0) and three months after this assessment (T1). Method: an observational, analytical and longitudinal study carried out in the cardiology outpatient clinics of two public hospitals in Ribeirão Preto, São Paulo. The sociodemographic and clinical data were collected through interviews and consultation of medical records. Self-care was assessed using the Brazilian version of the Self-Care of Heart Failure Index-SCHFI instrument. The data were analyzed by means of the Student’s t test and paired distribution (McNemar) with a significance level of 0.05. Results: we verified an increase in the mean scores of the three subscales of SCHFI (Maintenance, Management and Confidence), when comparing the values of T0 and T1, these differences being statistically significant (p<0.001). When comparing the positive changes in self-care actions over these months, we found statistically significant changes in the Maintenance (6 out of 10 items), Management (5 out of 6 items) and Confidence (4 out of 6 items) subscales. Conclusion: self-care for heart failure improved in the period between the first return after discharge and the end of three months of follow-up. Further studies are needed to verify the variables associated with improved self-care after hospitalization.

Heart failure (HF) is a clinical condition that requires comprehensive medical care, consisting of supervision by a cardiologist, primary care physician (PCP), nurse practitioner and nutritionist. A physiotherapist, pharmacologist play a no less important role, and psychological support for both the patient himself and his family can be equally helpful, which can positively influence the treatment process. A significant problem in the care of patients with HF are repeated hospitalizations, which can result from the progression of the disease itself, but also from suboptimal therapy, comorbidities or noncompliance. As each hospitalization worsens the patient's prognosis, but just as importantly, generates high costs, outpatient care options involving different groups of medical personnel are being sought to improve the prognosis and quality of life of HF patients, but also reduce the cost of care. Currently, 90% of the budget for HF care is consumed by hospitalizations. This article aims to show how the competencies and tasks of a nurse can be used in the care and education of a patient with HF. The role of the nurse in the outpatient care of patients with HF is emphasized by the guidelines of the European Society of Cardiology, resulting in the creation of the Education and Certification Program for Heart Failure Nurses in Poland under the auspices of the Polish Cardiac Society, the Heart Failure Association, the Supreme Chamber of Nurses and Midwives.

Purpose As insufficient adherence to recommendations is a major cause of poor health outcomes in patients with heart failure (HF), it is important to identify the factors that improve disease knowledge and self-care behaviors. We aimed to identify factors associated with HF knowledge and self-care behaviors in patients hospitalized due to acute decompensated heart failure (ADHF) in Poland. Patients and Methods This was a secondary analysis of data from a multi-center, prospective, non-randomized educational program named “The Weak Heart”, which was conducted in 14 cardiology centers in Poland. The level of HF knowledge was assessed using a questionnaire developed by the research team, and self-care behaviors were evaluated using the European Heart Failure Self-care Behaviour Scale (9-EHFScBS) questionnaire. Results The study included 259 patients admitted to the hospital with ADHF. The mean HF knowledge and 9-EHFScBS questionnaire scores were 8.25± 4.34 and 25.2±9.5, respectively. The patients’ level of education (P = 0.002), number of prior HF hospitalizations (P = 0.008), and previous disease education (P < 0.001) were independently associated with better HF knowledge. Age (P = 0.03) and disease education (P = 0.02) were independently associated with self-care behaviors. Conclusion The adherence to self-care recommendations among patients with ADHF is insufficient. Disease education positively influenced both HF knowledge and self-care abilities. To improve patients’ adherence to HF recommendations, well-designed models of education based on HF management guidelines should be implemented.
