Type III radical hysterectomy reported in 1974 by Piver, Rutledge, and Smith is considered worldwide by many as the standard surgical therapy for invasive cervical carcinoma stage IB and IIA. With the increasing number of robotic surgeries being performed for early stage cervical cancer worldwide, the purpose of the paper is to present our personal perspective of the 21st century role of Piver-Rutledge type III radical hysterectomy for stage IB cervical cancer in the era of robotic surgery using the da Vinci robot.

RESUMENEntendemos por endoscopia a aquellos procedimientos que permiten diagnosticar y tratar patología por medio de instrumentos con los que se observa el interior del cuerpo humano. Los primeros tratamientos por medio de endoscopia fueron realizados por Hipócrates. Luego a través de la historia son múltiples los precursores, pioneros y tantos otros que entregaron invenciones que permitieron llegar hasta el laparoscopio moderno. Convirtiéndose en una herramienta indispensable en la actual cirugía ginecológica mínimamente invasiva. Las nuevas tecnologías han permitido desarrollar la robótica, de la cual no han permanecido ajenos los procedimientos laparoscópicos. Así es como nace el robot Da Vinci, que más que una tecnología del futuro, ya constituye una realidad del presente en la cirugía laparoscópica ginecológica. PALABRAS CLAVES: Laparoscopia, cirugía mínimamente invasiva, cirugía robótica, Da Vinci SUMMARYWe understand by endoscope to those procedures that allow diagnosing and treated pathologies by means of instruments with which we observed in the interior of the human body. The first treatments by means of endoscope were made by Hipocrates. Soon through history the precursors, pioneers and so many others are multiple that gave inventions that allowed arriving until modern laparoscopy; becoming an indispensable tool in the present minimally invasive gynecological surgery. The new technologies have allowed developing the robotics technologies of which the laparoscopy procedures have not remained out. Thus the Da Vinci robot has born, who more than a technology of the future already constitutes a reality of the present in the gynecological laparoscopic surgery.

Objective: The aim of this study was to compare the surgical outcomes of robotic-assisted radical hysterectomy (RRH) with traditional laparoscopic radical hysterectomy (TLRH) for the treatment of early-stage cervical cancer in a large retrospective cohort of a total of 933 patients. Methods: We have enrolled 100 patients into the RRH and 833 patients into the TLRH group. The surgical outcomes include operating time, blood loss, transfusion rate, pelvic lymph node yield, hospitalization days, duration of bowel function recovery, catheter removal before and after 3 weeks, conversion to laparotomy, and intraoperative and postoperative complications. Follow-up results were also analyzed for all patients. Results: Both groups have similar patient and tumor characteristics but patients with a larger lesion size were preferably enrolled in the TLRH treatment group. The treatment with RRH was generally superior to TLRH with respect to operating time, blood loss, length of hospitalization, duration of bowel function recovery, and postoperative complications. On follow-up of patients, there were no relapses reported in the RRH group compared with 4% of relapse cases and 2.9% of deaths because of metastasis in the TLRH group. No conversion of laparotomy occurred in the RRH group. No significant difference was found with respect to intraoperative complications and blood transfusion between both groups. Conclusions: The results from this study suggest that RRH is superior to TLRH with regard to surgical outcome and may pose a safe and feasible alternative to TLRH. The operating time and lymph node yield is acceptable. Our study is one of the largest single-center studies of surgical outcomes comparing RRH with TLRH during cervical cancer treatment and will significantly contribute to the safety of alternative treatment options for patients. Furthermore, the difference detected between TLRH and RRH group is further strengthened by the great expertise of the surgeon performing laparoscopic surgeries.
