Robot-Assisted Laparoscopic Excision of Symptomatic Retrovesical Cysts in Boys and Young Adults

Abstract: In the management of retrovesical cystic anomalies robot-assisted laparoscopic excision affords a natural extension of conventional laparoscopy with the additional advantages of 3-dimensional vision and ease of instrument control.

“…Five cases of robotic SV excision and one single-center study with 6 patients including 4 with SV cyst excision, have been reported in the literature to date. [9][10][11][12][13][14] The estimated blood loss ranged from 5 to 50 cm 3 , operative time ranged from 120 to 231 min, and in most cases the patient was discharged the first or second postoperative day; the only reported complications were transient brachial palsy due to patient positioning 13 and a single case of a self-limiting small pelvic hematoma. 9 We perform RARP at our institution using the posterior approach to the dissection of the seminal vesicles and vasa deferentia, 4 which makes the retrovesical anatomy familiar to the surgeon.…”

“…The differential diagnosis of a seminal vesicle cyst includes enlarged prostatic utricle, Müllerian duct cyst, intraprostatic cyst, ejaculatory duct cyst, hydronephrotic pelvic kidney or ureters, bladder diverticula, or ureterocele. However, a seminal vesicle cyst is nearly always unilateral with a few documented cases of bilateral cysts reported, lateral to the midline, and unilocular (Sheih et al 1998;Razi and Imani 2000;Hong et al 2011). …”

“…Transrectal aspiration, cystoscopy with transurethral resection of the ipsilateral ejaculatory duct, and intravesical cyst unroofing have been described, but with limited durable success (Cherullo et al 2002). Surgical excision may be approached open from a retropubic approach or laparoscopic, or robotically from a transperitoneal approach (Hong et al 2011;Zhang et al 2012). …”
