Robot-assisted brachytherapy of the bladder with long distance support using video conferencing

Abstract: Purpose: The frontline treatment for localized muscle-invasive bladder carcinoma (MIBC) is radical cystectomy. However, a significant percentage of this population is elderly with either severe co-morbidities or suboptimal general health, increasing the per-and post-operative risk when undergoing a radical cystectomy. Conservative treatment options have been implemented such as robot-assisted laparoscopic brachytherapy (RALB), a minimally invasive therapeutic approach ensuring excellent results in terms of loc… Show more

“…RAB is available in only a few selected centres even in the western world (we can see the publications of laparoscopic and robotic brachytherapy are from a few selected centres only). We found an interesting article that RAB for bladder cancer has been guided by tele conferencing, so training in centres with baseline expertise in robotic surgery brachytherapy may not be that difficult ( 17 ). The Da Vinci robot manufactures have come with training facilities like dual consoles and simulators ( 18 ).…”

“…Both the robot-assisted procedure and the use of HDR were adopted at NKI, Amsterdam, where in 2018, Bosschieter [49] described the implantation procedure pioneered in Arnhem. The method was also adopted in Lisbon, where Mascarenas [50] showed feasibility of the treatment.…”

“…Radical cystectomy is considered the treatment of choice for muscle-invasive bladder carcinoma[ 1 - 6 ]. Surgery may not be possible, however, for patients with locally advanced tumors or for those considered medically unfit[ 7 , 8 ]. External beam radiotherapy (EBRT) is a noninvasive and efficient treatment modality[ 9 - 11 ], although it is based on very limited literature consisting mainly of small retrospective series of patients not eligible for curative treatment[ 12 - 14 ].…”
