Risk stratification model, including preoperative serum C-reactive protein and estimated glomerular filtration rate levels, in patients with upper urinary tract urothelial carcinoma undergoing radical nephroureterectomy

Abstract: This study indicates that preoperative hematological markers, such as low eGFR and high CRP levels predict a poor survival in patients with UTUC. Our risk stratification model may provide physicians with useful prognostic information for identifying patients who may be candidates for multimodal treatment.

“…In general, creatinine levels are influenced by lower muscle mass, creatine kinase activity, glomerular filtration, and renal secretion . Some studies examined the effect of kidney function on inpatient mortality, but data on cancer patients are scarce . The current evidence in the literature does not show a causal association between creatinine levels and mortality in cancer patients, but there are some reasonable thoughts about the underlying pathomechanism.…”

“…Similarly, markers of kidney dysfunction were also found to predict inpatient mortality, but data on cancer mortality is scarce . Serum creatinine is a byproduct of muscle metabolism that is excreted by the kidneys and therefore a commonly used indicator of renal function.…”

“…One possible explanation is that chronic irritation which was caused by diminished urinary washing effect and accumulation of oncogenic metabolized nephrotoxic substances in the upper urinary tracts in CKD patients may contribute to a more aggressive pattern of UTUC [19]. In addition, CKD is also a proinflammatory condition [20], and there are several studies that have shown that markers of inflammation, such as C-reactive protein (CRP), have also been associated with oncological outcomes in patients with UTUC who underwent RNU [21]. Morizane et al [21] reported that high preoperative CRP level and low preoperative eGFR ( < 50 mL/min/1.73m…”
