Risk of non-Hodgkin lymphoma subtypes in HIV-infected people during the HAART era

Abstract: Objective HIV-infected people have greatly elevated risk of non-Hodgkin lymphoma (NHL), particularly the AIDS-defining NHL subtypes: diffuse large B-cell lymphoma, Burkitt lymphoma and primary lymphomas arising in the central nervous system. The goals of this analysis were to comprehensively describe risks of NHL subtypes, especially those not well studied, among HIV/AIDS patients; examine risks specifically in the HAART era; and distinguish risks in HIV-infected individuals prior to diagnosis with AIDS. Des… Show more

“…We observed no HIV in BL, which is somewhat inconsistent with associations seen in other sub‐Saharan African populations,3, 4, 18, 19 and the fact that BL is a recognized AIDS‐defining subtype of NHL in adults 17. Of note, the endemic variant of BL in sub‐Saharan Africa mainly affects children and all cases occurred in persons <18 years old in Rwanda.…”

“…We saw no cases of primary effusion lymphoma, also considered to be strongly associated with HIV 15. Nor did we find any associations with T‐cell NHL subtypes, consistent with large studies in high‐income settings 16, 17…”

“…From 2007 to 2010, 13 AMCs enrolled 34 patients (Table 2), all but 2 of whom were high risk. Four had evidence of leptomeningeal disease, none had parenchymal CNS disease, 88% were male, 65% were white, 15% were African American, and 24% were Hispanic.…”

“…[10][11][12] BL was more recently estimated at 10%, although this study included 31% NHL unspecified. 13 Patients with HIV-BL closely resemble the general BL population in terms of stage, marrow involvement (33% to 35%), CNS involvement (17% to 19%), 14 and histology. 15 Prior to HAART, combination chemotherapy was less successful for patients with HIV-associated NHL than for HIV-negative patients with similar NHL histopathology.…”

“…The advent of HAART not only effected a decreased incidence of AIDS-related NHLs but also underpinned improved patient survival attributed to decreased co-morbid infections, improved tolerance to chemotherapy and increased remission rates [21]. Notwithstanding the impact of HAART on AIDS outcomes, the risk of NHL remains elevated in the AIDS context [22]. Whilst the most common lymphomas in the HAART era are BL and DLBCL [23], the spectrum of HIV-associated lymphomas has expanded to include those that occur commonly in the general population and those that occur with heightened propensity in HIV-infected patients ( In contrast to that in HIV-infected adults, B-cell NHLs are the most common malignancy in HIV-infected children [27].…”
