While baker's asthma has been well described, various asthma phenotypes in bakery workers have yet to be characterised. Our study aims to describe the asthma phenotypes in supermarket bakery workers in relation to host risk factors and self-reported exposure to flour dust.A cross-sectional study of 517 supermarket bakery workers in 31 bakeries used a questionnaire, skin prick tests, and specific immunoglobulin E to wheat, rye and fungal aamylase and methacholine challenge testing.The prevalence of probable occupational asthma (OA, 13%) was higher than atopic (6%), nonatopic (6%) and work-aggravated asthma (WAA, 3%) phenotypes. Previous episodes of high exposure to dusts, fumes and vapours causing asthma symptoms were more strongly associated with WAA (OR 5.8, 95% CI 1.7-19.2) than OA (2.8, 1.4-5.5). Work-related ocular-nasal symptoms were significantly associated with WAA (4.3, 1.3-13.8) and OA (3.1, 1.8-5.5). Bakers with OA had an increased odds ratio of reporting adverse reactions to ingested grain products (6.4, 2.0-19.8).OA is the most common phenotype among supermarket bakery workers. Analysis of risk factors contributes to defining clinical phenotypes, which will guide ongoing medical surveillance and clinical management of bakery workers.

StreszczenieNieżyt błony śluzowej nosa jest przewlekłą zapalną chorobą górnych dróg oddechowych cechującą się znacznym rozpowszechnieniem i złożoną patogenezą. Termin 'nieżyt nosa związany z pracą' (work-related rhinitis -WRR) jest pojęciem obejmującym zarówno nieżyt nosa wywołany pracą -zawodowy nieżyt błony śluzowej nosa, jak i nieżyt nosa zaostrzony pracą. Zawodowy nieżyt nosa jako jednostka chorobowa jest także rozpatrywany w kontekście orzecznictwa jako schorzenie alergiczne związane z ekspozycją zawodową. Nieżyt nosa związany z pracą jest jednostką chorobową o zróżnicowanej epidemiologii zależnie od grupy zawodowej i rodzaju narażenia, z drugiej strony dane dotyczące rozpowszechnienia mogą być niedoszacowane ze względu na brak jednolitych kryteriów diagnostycznych. Niniejsza praca ma na celu przegląd danych dotyczących zagadnień związanych z patogenezą, epidemiologią, diagnostyką i postępowaniem, w tym również w kontekście orzecznictwa, wśród osób z nieżytem nosa zwią-zanym z pracą. W niniejszej pracy omówiono także znaczenie tego schorzenia w medycynie pracy, szczególnie w aspekcie opieki profilaktycznej nad pracownikiem, ogólne zasady dobrej praktyki w prewencji pierwotnej i wtórnej WRR oraz niezbędne kierunki zmian w orzecznictwie w przypadku zawodowego nieżytu nosa. Work-related rhinitis (WRR) can be divided into occupational rhinitis (OR) and work-exacerbated rhinitis (WER). It is not only considered as a disease entity but also in the context of medical certification as the allergic disease associated with occupational exposure. Epidemiology of work-related rhinitis has been found to vary depending on the occupation and specific exposure, on the other hand the prevalence data may be underestimated due to the lack of uniform diagnostic criteria. This paper reviews the issues comprising the pathogenesis, epidemiology, diagnosis and treatment of patients with work-related rhinitis. It also discusses the significance of the disease in occupational medicine, particularly in terms of preventive worker care, general principles of good practice in primary and secondary WRR prevention and the necessary directions of changes in medical certification in the cases of occupational rhinitis. Immunotoxicity can manifest as an exaggerated immune response to a normally innocuous substance and lead to hypersensitivity or allergic reactions in the lung. Allergens are encountered environmentally or in the workplace and can be low‐ and high‐molecular‐weight substances. Allergic asthma and allergic rhinitis are examples of allergic reactions in the lung with immediate, late and chronic phases contributing to respiratory tract pathology. Mechanistically, allergic asthma and rhinitis reflect a combination of antibody and cell‐mediated immune reactions involving IgE antibody to allergen in the early stages and Th2 cell involvement as the disease progresses to late‐phase reactions and chronic allergic inflammation. Multiple cell types and mediators are involved in the inflammatory response in the lung. Mechanistic differences leading to the asthma/rhinitis phenotype have been noted with different allergens, primarily low‐molecular‐weight substances. Xenobiotics that are not allergens themselves may enhance the immune response to allergens or exacerbate pre‐existing asthma and airway hyper‐responsiveness. The complexity of the allergic response in terms of humoral and cell‐mediated immunity, as well as potential differences in mechanisms depending on the allergen, present a very challenging scenario for the toxicologist to predict the exposures that will result in allergic rhinitis or asthma.
