Risk of all-cause mortality associated with nonfatal AIDS and serious non-AIDS events among adults infected with HIV

Abstract: Objectives Among patients with HIV, the risk of death associated with different AIDS events has been quantified, but the risk of death associated with non-AIDS events has not been examined. We compared the risk of all-cause mortality following AIDS versus serious non-AIDS (SNA) events in SMART and ESPRIT. Design Data from 9,583 HIV-infected participants, 5,472 with CD4+ >350 cells/mm3 enrolled in SMART and 4,111 with CD4+ ≥300 cells/mm3 enrolled in ESPRIT were analyzed. Methods Cumulative mortality 6 month… Show more

“…With this, there has been a shift in distribution of causes of death toward non‐AIDS‐defining events (NADEs) 8, 9, 10, 11, 12. Factors contributing to non‐AIDS mortality include immunodeficiency 13, 14, ART toxicity 15, 16, increasing age 17, 18, 19, 20, 21, 22, and lifestyle factors such as tobacco use and obesity 18, 21, 23.…”

“…CVD-related mortality is anticipated to increase as HIV-infected patients initiate antiretroviral therapy (ART) at CD4+ counts >200 and opportunistic infections become uncommon [35]. In sub-Saharan Africa, there are numerous challenges in the diagnosis and control of hypertension (HTN), including low awareness among patients and providers [6], overcrowded clinics, and limited opportunities for HTN screening [7].…”

“…Most studies of AIDS have analyzed the factors associated with therapeutic failure, such as death, opportunistic disease, detect Viral Load (VL) or low levels of immunity, using semi parametric Cox models (Pradeep et al, 2010;Muralidhar et al, 2010;Keiser et al, 2010;Koethe et al, 2010;Neuhaus et al, 2010). However, it is also important to consider chronicity and the relatively slow progression of the disease and to assess the multiple immune states resulting from the effect of the virus on the human immune system.…”
