Risk Model of Thoracic Aortic Surgery in 4707 Cases From a Nationwide Single-Race Population Through a Web-Based Data Entry System

Abstract: Background-The objective of this study was to collect integrated data from nationwide hospitals using a web-based national database system to build up our own risk model for the outcome from thoracic aortic surgery. Methods and Results-The Japan Adult Cardiovascular Surgery Database was used; this involved approximately 180 hospitals throughout Japan through a web-based data entry system. Variables and definitions are almost identical to the STS National Database. After data cleanup, 4707 records were analyzed… Show more

“…Furthermore, the goodness of fit of EuroSCORE II seems to be adequate for risks levels of <30%, beyond which a systematic tendency to risk overestimation is likewise observed (27). Therefore, we can state that current scores do not meet the requirements to address the complex decision-making process characterizing multiple open heart surgery scenarios, including AAS (28)(29)(30).…”

“…1,2 Thus, risk stratification of patients prior to a surgical procedure is useful and important for determining patient management and improving quality control. 1 Although prognostic risk models for cardiac and aortic surgery, such as EuroSCORE II and JapanSCORE, 3-5 have been introduced into current clinical practice, there is still room for improvement in terms of their predictive ability. 6 Endothelial function is known to be associated with maintenance of vascular homeostasis and regulation of the coagulation system.…”

“…The most recent annual number of submitted procedures are 49,507 in JACVSD and 10,835 in JCCVSD. Twenty frequently cited papers dealing with topics such as risk models of isolated coronary bypass surgery [8], thoracic aortic surgery [9], and valve surgery [10] have been published in indexed international journals.…”
