Risk Factors for Thrombocytopenia in Adult Chinese Patients Receiving Linezolid Therapy

Abstract: The incidence of LZD-related thrombocytopenia in the Chinese population is much higher than that suggested by the drug instructions. Low pretreatment platelet count, low body weight, low serum albumin concentration, long-term drug administration, advanced age, renal insufficiency, and concomitant use of caspofungin, levofloxacin, and meropenem have been identified as risk factors. Although predictors have been proposed for use in clinical practice to screen for patients at high risk who require intensified mon… Show more

“…Many retrospective studies have described various risk factors for linezolid-induced thrombocytopenia, with the main potential risk factors including renal impairment, [4][5][6][7][8][9][10][11] prolonged treatment duration, 4,5,10) hypoalbuminemia, 10) and lower body weight. 10,11) Hyponatremia was observed in 7% of patients treated with linezolid in a phase III clinical trial in Japan. 12) Hyponatremia is the most common impairment of body fluid and electrolyte balance encountered in the clinical setting.…”

“…We thought that usage of these drugs also affected the development of thrombocytopenia in our second patient. Due to this, we advise that more attention should be paid to the development of thrombocytopenia when LZD is used with other anti-infective medications as in our second case 13 .…”

“…We believed that this factor also affected the development of thrombocytopenia in our patients. The concomitant use of meropenem with LZD and use of vancomycin prior to LZD may worsen thrombocytopenia 12,13 . Vancomycin was given to our second patient prior to LZD and meropenem was given concomitantly.…”

“…10 Thrombocytopenia is a common adverse effect in adult patients and the incidence of linezolidrelated thrombocytopenia has been reported about 15-50%. 7,11,12 The other common drugrelated side effects reported with linezolid therapy were diarrhea, nausea, headache, neuropathy, and abnormal liver function tests. 13 These adverse effects were mostly detected in patients receiving linezolid for longer than 2 weeks.…”
