Risk Factors for the Development of Hypothyroidism After Hemithyroidectomy

Abstract: Setting: A tertiary care academic medical center. Patients: The study included 90 patients who underwent a hemithyroidectomy from 1999 to 2004. Main Outcome Measures: Hypothyroidism was defined as a serum thyrotropin level greater than 6.0 mIU/L at least 8 weeks after hemithyroidectomy. All patients were analyzed for age, sex, surgical indications, preoperative and postoperative thyrotropin levels, weight of resected specimen, final pathologic analysis, and length of followup. Multivariate analysis was perform… Show more

“…In all cases with a preoperative diagnosis of carcinoma, the recommended treatment is total thyroidectomy 11,18,19 . This trend is driven by the more frequent recognition of postpartial thyroidectomy hypothyroidism, both in our experience (not yet published) and in several other analyzes 19,[23][24][25][26][27] and also by the advantage of enabling the adjuvant radioiodine therapy when indicated, and the more accurate use of serum thyroglobulin and whole body scan with I 131 during postoperative follow-up, without increased risk of complications 28 . The treatment of nodular thyroid disease in our Department routinely involves intraoperative frozen biopsy.…”

“…Therefore, patients who are postoperatively euthyroid rarely go on to develop hypothyroidism, making yearly evaluations by measuring serum TSH concentrations sufficient. Thus, most thyroid function should be monitored once yearly for life (McHenry & Sulsarczyk, 2000;Miller et al, 2006).…”

“…Miller, et al (2006) Recommendation is to obtain a postoperative TSH measurement 8 to 12 weeks after surgery, followed by the measurement of TSH levels at 6 months and 12 months after surgery. If the TSH level is normal at 12 months, biannual to annual determination of TSH levels was encouraged unless symptoms of hypothyroidism manifest.…”

“…4 Anatomically subglottis, paraglottic space, cricothyroid space, thyroid cartilage, and cricothyroid membrane lie very close to each otherwhich helps to explain further regarding the thyroid gland involvement and extension in advanced stage laryngeal tumors. 7 Hemithyroidectomy still poses a risk of hypothyroidism with an incidence of 5.0% to 41.9% in all patients underwent hemithyroidectomy. The etiology causing hypothyroidism in hemithyroidectomy patients still remain uncertain 9 . In our daily life, in order for us to remain euthyroid, thyrocytes produced by one thyroid lobe is enough and with that believe many doctors choose to do hemithyroidectomy as to reduce the usage of postoperative thyroid hormone therapy.…”
