Risk Factors for Poor Treatment Outcomes in Patients with MDR-TB and XDR-TB in China: Retrospective Multi-Center Investigation

Abstract: BackgroundThe treatment of patients with MDR- and XDR-TB is usually more complex, toxic and costly and less effective than treatment of other forms of TB. However, there is little information available on risk factors for poor outcomes in patients with MDR- and XDR-TB in China.Methodology/Principal FindingsWe retrospectively analyzed the clinical records of HIV-negative TB Patients with culture-proven MDR- or XDR-TB who were registered from July 2006 to June 2011 at five large-scale Tuberculosis Specialized Ho… Show more

“…A literature review of comorbid conditions showed that having any comorbid condition was associated with unsuccessful treatment outcomes, mainly death. Vasankari et al found that the presence of immunosuppresion and malignity was associated with death, while the presence of any comorbid disease [7], diabetes mellitus and malignity [9], and diabetes mellitus [12][13][14] were associated with unsuccessful outcomes.…”

“…16 Being underweight, with a BMI  18.5 kg/m 2 , in patients with MDR-TB has been found to be a risk factor for unfavourable outcomes, particularly mortality. 3,12,17,18 Being underweight is a manifestation of severe disease and possibly poor socio-economic circumstances, and as it also impairs host immunity against mycobacteria, it is not surprising that there are higher rates of mortality in such patients. HIV infection, which may be associated with malnutrition, has also been reported as a risk factor for death in patients with MDR-TB, 19,20 although observational cohort studies have shown that timely antiretroviral therapy can improve survival.…”

“…Resistance to front-line therapies necessitates treatment with up to five or six second-line agents that are poorly tolerated, and treatment success is only achieved in 40-70% of patients (5). Failure to cure drug-resistant disease leads to acquisition of further resistance with a progressively poorer prognosis for these patients, thus fueling an emerging epidemic of drug-resistant disease that threatens to overwhelm fragile health care systems in developing countries (6).…”
