Risk factors for in-hospital mortality in cirrhotic patients with oesophageal variceal bleeding

Abstract: Cirrhotic patients with OVB and MELD of 18 or more, platelets of 100/ml or less and requiring transfusion of 2 or more units of PRBC were at an increased risk of IHM. Overall, the logistic regression model correctly predicts 82.2% of IHM patients.

“…Several studies have have highlighted that variceal rupture risk increases with the increase in severity of the liver disease. 7,9,10 The most frequent aetiology of upper GI bleed in cirrhosis was rupture of esophageal varices in eighty (66.7 %) patients in our study. Variceal bleed accounted 57.7%, 73% and 50% of acute UGI bleed in cirrhotics in their studies by Svoboda et al 6 ,Romcea et al 7 and Olajide et al 8 respectively.…”

“…[113][114][115] MELD score has been shown to be an accurate predictor of outcome after VH. 83,116,117 MELD >18 was a predictor of rebleeding within first 5 days and overall mortality at 6 weeks.…”

“…При использовании традиционного лапаротом-ного доступа наиболее эффективным, радикальным, а также щадящим оперативным вмешательством с наименьшим количеством п/о осложнений, пост-ваготомных нарушений и неудовлетворительных отдаленных результатов являлись ОСО с иссечением язвы в сочетании с СПВ или КСПВ [5,7]. Однако применение СтВ и СВ приводило к большему числу функциональных нарушений.…”
