Objectives: The main aim of the study is to identify the different risk factors, prescribing patterns of medication and rationality of the prescription by observing the management of stroke patients carefully. Methodology: A prospective and observational study was carried out for a period of 6 months in an in-patient department of a tertiary care hospital. Patients above 18 years of age, patients of either sex and patients who previously had stroke were included. Children below 18 years, pregnant women, AIDS patients, patients unable to respond to verbal questions with no caretakers and patients with head injury were excluded. Results: A total of 100 prescriptions were analyzed during the 6 months study period. This was found to be higher in males (79) and females (21). The higher risk with notable occurrence in the age group of 60-79 years (43%). Out of these study populations, (68%) experienced Ischemic stroke and (32%) experienced Hemorrhagic stroke. The major risk factors were Hypertension (62%), alcoholics (41%). Smoking (33%) and Diabetes (24%). Among the 100 prescriptions, Atorvastatin was highly prescribed (96) followed by Pantoprazole (92), Mannitol (64). Out of 100 prescriptions, (67) were rational and (33%) were irrational. Conclusion: The prescribing patterns of drug should be based on specificity and severity of stroke in order to facilitate rational use of drugs providing optimal care. Early management of hypertension and diabetes may reduce risk of stroke. Cessation of smoking and alcohol place a key role in prevention of stroke. Objectives: This research is to measure the prevalence rate of antiplatelet resistance in ischemic stroke patients and measure the vascular risk factors associated with antiplatelet resistance in patients with ischemic stroke. Methods and Subjects: This was a cross-sectional study with the number of respondents in this study amounted to 155 patients who all had ischemic stroke disease at Bethesda Hospital in Yogyakarta Indonesia used stroke registry to complete the data of the risk factors. VerifyNow method is used to measure the responsiveness of antiplatelet therapy. Results: Among the 155 patients with ischemic stroke, 45 were women (29%), 110 were men (71%), and the elderly (age more than 60 years old) in 81 patients. In total 155 patients with ischemic stroke, 106 of them have hypertensive, with diabetes are 19 patients, dyslipidemia is 90 patients, and ischemic heart disease in 13 patients. The prevalence of antiplatelet resistance in risk factors, for age more than 60 years, is 21 patients (25%, RR=1.06, *p=0.96), in diabetes is 7 patients (36%, RR=1.17, **p=0.74), dyslipidemia is 19 patients (21%, RR=0.68, ***p=0.24), and ischemic heart disease is four patients (30%). Among 127 patients, 22% (28 patients) had aspirin resistance, while from 42 patients, 26.2% (11 patients) were resistant to clopidogrel. Conclusion: Antiplatelet resistance is common in ischemic stroke patients. One of five patients treated with antiplatelet showed non-responsiveness. Vascular risk factors do not increase the risk of antiplatelet resistance in ischemic stroke patients.

Two main strategies have been suggested for the treatment of ischemic stroke including reperfusion and neuroprotection. Citicoline is one of neuroprotector agent that commonly used in ischemic stroke. A systematic review of citicoline focusing only on ischemic stroke has not yet performed. This systematic review aimed to identify the effectiveness of citicoline in patients with ischemic stroke history. A systematic research was performed using PubMed and Cochrane Library as the database. Keywords were: "citicoline and stroke", "citicoline and ischemic stroke", and "randomized controlled trial citicoline". The inclusion criteria were: (i) the trials conducted between 2007 to 2017, (ii) written in English, (iii) the studies performed in human, (iv) patients had ischemic stroke history, (v) citicoline was the main drug assessed in the study. The exclusion criteria i.e.: (i) the study was not a randomized controlled trial (RCT) and (ii) the full text was not available. The quality of RCT was determined by using Jadad score. A literature search revealed 541 journals from PubMed and Cochrane Library. After screening the title and abstract, adjusting for inclusion and exclusion criteria, adjusting for the irrelevant topic, and duplication the final result was 4 RCT studies. All studies had a good quality, indicated by Jadad score of 3 or more. Three studies conclude that no statistically significant difference in treatment outcome between citicoline and other groups. The last study revealed citicoline is effective in preventing post-stroke cognitive impairment.
