Right heart dilatation in adult congenital heart disease: imaging appearance on cardiac magnetic resonance

Abstract: ABSTRACT. Imaging morphology and function of the right heart is of paramount importance in patients with adult congenital heart disease, since right ventricular dysfunction is associated with adverse cardiac events. Cardiac MRI has been shown to be a powerful tool for the non-invasive precise assessment of right ventricular and valvular dysfunction. Differential diagnoses of congenital heart disease characterised by, or combined with, right heart dilatation are diverse and necessitate a systematic approach.

“…These patients have comparatively higher prevalence of co-morbid diseases including diabetes mellitus, stroke, systemic hypertension, chronic lung diseases atherosclerosis and coronary heart diseases [21][22][23] . Longstanding left to right shunt at the atrial level further results in a progressive atrial stretch and right ventricular dilatation which in turn eventually leads to tricuspid insufficiency [24] . The left heart may also be influenced by way of increased atrial pressure, chronic volume under load, and co-morbid diseases like systemic hypertension or coronary heart disease [6] .…”

“…RV dilatation is a condition mostly observed in the presence of a significant left to right shunt. Infrequent causes include tricuspid valve and pulmonary valve (PV) regurgitation, Ebstein's anomaly and postoperative insufficiency after tetralogy of Fallot repair . The following case demonstrates a rare cause of right ventricular dilatation in a patient whose previous private practice echocardiogram failed to diagnose the RV dilatation.…”
