Right foot congenital infantile fibrosarcoma treated only with chemotherapy

Abstract: Congenital infantile fibrosarcoma (CIF) is a rare tumor in childhood. The 5‐year survival rate for CIFs is high and has been reported between 84% and 93%, but limb‐amputation/disarticulation is still a major problem. We report the case of a male newborn with a mass in his right foot. X‐ray and MRI revealed a mass destroying all tarsal, metatarsal, and phalangeal bones. The patient was treated only with VAC chemotherapy and is able to walk normally. Pediatr Blood Cancer 2010;54:618–620. © 2009 Wiley‐Liss, Inc.

“…Another study showed a rate of 21 % [14]. Lymphadenectomy is usually not warranted, because IFS seldom metastasizes to lymph nodes, but two case reports have described regional lymph node metastasis [17, 18]. We did not observe any regional lymph node metastasis, except for positive omental lymph nodes in the patient with a retroperitoneal lesion.…”

“…Patients have been treated with neoadjuvant chemotherapy [13, 23] or chemotherapy alone [18, 21, 22, 24]. Neoadjuvant chemotherapy should be used for extensive lesions when complete surgical excision during an upfront procedure is not possible or would result in amputation [20, 25].…”

“…We have reviewed 107 cases reported in the literature in children under one year of age until 2010. [1,2,[4][5][6][7][8][9][10] About 40% of CIFS are diagnosed at birth, and 60% before 3 months. Boys are more often affected than girls (sex-ratio = 3/1 to 4/1) [11] .…”

“…[20] Recently, Demir et al . [24] were able to successfully treat a newborn with CFS of the right foot that had destroyed all tarsals, metatarsals and phalangeal bones, with only VAC-based chemotherapy. Spontaneous regression has also been reported in some neonatal and pediatric cases of CFS.…”
