Rhabdomyolysis and Compartment Syndrome with Coadministration of Risperidone and Simvastatin

Abstract: We report a case of rhabdomyolysis and acute compartment syndrome of the lower extremity in a schizophrenic patient taking risperidone following the addition of simvastatin to treat hyperlipidemia. We suspect that disrupted drug metabolism, resulting from interactions with cytochrome P450 enzymes, rapidly elevated drug plasma levels, which then led to muscle toxicity. Clinicians who pharmacologically treat medical comorbidities in patients receiving atypical antipsychotics must be proactive in anticipating pot… Show more

“…risk-scoring system for patients with SMI (Taxis et al 2008;De Hert et al 2009). Even if an adapted risk score system would be available for patients with SMI, implementation of somatic treatment regimens may be complicated by pharmacokinetic inter actions between statins and antipsychotics (Baptista et al 2004;Webber et al 2004) or by adverse somatic eff ects induced by β -adrenergic antagonists and thiazide diuretics, such as weight gain and glucose dysregulation (Sharma et al 2001;Elliot and Meyer 2007).…”

“…No studies have yet been published on the efficacy of statins in the treatment of antipsychotic-related dyslipidemias, although a few case reports on individual patients have described some possible side effects for the co-administration of SGAs (risperidone, quetiapine) with statins (Furst et al, 2002;Giner et al, 2002;Webber et al, 2004). Further, recent guidelines suggest frequent monitoring of cardiovascular risk factors in patients with schizophrenia, but this sort of monitoring begets the question of how to treat such disorders, especially in cases that have an unfavorable lipid profile.…”
