Multiple pregnancies are considered as high-risk pregnancies due to the increased associated maternal and fetal morbidity and mortality. The incidence of multiple pregnancy varies from region to region, with a very high incidence recorded in Nigeria and other African countries. The aim of this study was to determine the prevalence and the feto-maternal outcome of multiple pregnancies. This was a retrospective study of all pregnant women who had multiple gestation at the University of Port Harcourt Teaching Hospital from January 1, 2012 to December 31, 2016. Data was obtained from the case notes and the special care baby unit (SCBU). The data obtained was entered into a spread sheet and analyzed using SPSS 22. Results are presented in simple percentages and tables. The P value was set at < 0.05. During the period under review there were 9,895 deliveries, of which 287 were multiple pregnancies, giving a multiple pregnancy rate of 29/1000 deliveries. The mean age of the women was 29.0 ±4.3 years, while the mean parity was 1.6± 1.5. Majority (85%) of the pregnancies were achieved spontaneously, while 15% were achieved through assisted reproductive technology. Twin pregnancies made up 92.1% of multiple gestations. The most common antenatal complication was pregnancy induced hypertension (22%). The caesarean section rate was 71%, the most common indication for caesarean section was abnormal lie and presentation of the leading twin (26.3%). About half (53%) of the babies were delivered preterm, with a mean gestational age at delivery of 35.1± 1.5 weeks. Of the 558 babies delivered, 52.3% had low birth weight. There were 43 perinatal deaths, giving a perinatal mortality rate of 77/1000 total births. Perinatal mortality was significantly associated with prematurity (0.0001) and booking status (0.0001). There were three maternal deaths, giving a maternal mortality ratio of 556.04/ 100,000 live births. All the maternal deaths were in unbooked patients. Multiple pregnancy is a high-risk pregnancy, associated with a high rate of perinatal and maternal morbidity and mortality. It is recommended that women with multiple pregnancy should have supervised antenatal care and delivery in a facility with good obstetric and neonatal intensive care services.

Background and objectives: The study aimed to analyze the prevalence, trends, and outcomes of twin pregnancies in Ile-Ife, Nigeria, over two distinct periods. Materials and methods: This research, based on a 14-year retrospective cohort study, scrutinized twin births occurring in two-time frames: recent years (2012–2018; Period II) and the recent past (2005–2011; Period I) at a University Teaching Hospital in Ile-Ife, Nigeria. The inclusion criteria were limited to twin births, excluding singleton and higher-order gestations. Outcomes were evaluated based on several parameters, including mode of delivery, birth weights, fetal gender combinations, APGAR scores, perinatal mortality, and maternal complications. Data analysis was conducted using the 26 th version of Statistical Package for the Social Science, with a significance threshold of p < 0.05. Results: The study documented a stable prevalence of twin gestations, registering at 20.7 per 1,000 births without a significant discrepancy between the two time periods (21.7‰ versus 19.7‰; p = 0.699). Individuals from the Yoruba tribe predominantly featured in both cohorts, showing no considerable variation between the two time periods [83 (95.4) vs. 120 (99.2); p -value = 0.116]). The data exhibited recurrent instances of caesarean delivery (65.6% vs. 50.2%, p = 0.119), vertex-vertex presentation (38.0% vs. 44.7%, p = 0.352), and differing sex combinations (33.3% vs. 38.0%, p = 0.722) across both time frames. Twin II neonates born through Caesarean section were more frequently admitted to neonatal intensive care units than Twin I (5.1% versus 4.6%; p = 0.001). The recent years witnessed a surge in preterm labor complications, notably higher than the earlier period (17.1% versus 7.8%; p = 0.008). Conclusion: The prevalence of twin births in Ile-Ife, Nigeria, demonstrates a fluctuating decline. To comprehensively understand the dynamics of twin births in the region, there is a pressing need for expansive, community-centric research in southwest Nigeria.

Background: Multiple pregnancy is a high-risk pregnancy associated with a higher maternal and perinatal complications compared to singleton pregnancy. Twinning is the commonest form of multiple pregnancy, and its prevalence varies across the globe, with lowest value in Japan and highest in Nigeria.Methods: The aim of the study was to review twin births in Rivers State University Teaching Hospital (RSUTH), to determine its prevalence, trend, and management outcome.Results: During the period under review, there were 13516, and 263 twin births, giving the prevalence of twinning in RSUTH as 19.5 per 1000 live births or 1 in 51 deliveries. The rate of twinning increased from 7.6 per 1000 deliveries in 2015 to 35.7 per 1000 deliveries in 2019. Mean age of patients was 31 SD 4.82 years, 95% CI: 30.42, 31.59, and modal age group was 30-34 years. Patients’ parity ranged from 0 to 7 with modal parity being para-1. Majority (51.7%) had secondary level education, and were Christians. Preterm delivery rate was 62.4%. The mean gestational age at delivery was 35.7 SD 2.9 weeks, 95% CI: 35.3, 36.0. The mean birth weights of the first and second twins were 2.39 SD 0.67 and 2.30 SD 0.69 respectively. Majority (92%) of the foetuses were alive at birth. Still birth and perinatal mortality rate were 4.2 and 7.8 per 1000 live births. There was no case of maternal mortality.Conclusions: The rate of twining in our setting is high, with an increasing trend. Most of the parturient had preterm delivery, and improved perinatal care services would ensure best perinatal outcome.
