Review of the Effectiveness of Capsaicin for Painful Cutaneous Disorders and Neural Dysfunction

Abstract: Capsaicin is effective for psoriasis, pruritus, and cluster headache; it is often helpful for the itching and pain of postmastectomy pain syndrome, oral mucositis, cutaneous allergy, loin pain/hematuria syndrome, neck pain, amputation stump pain, and skin tumor; and it may be beneficial for neural dysfunction (detrusor hyperreflexia, reflex sympathetic dystrophy, and rhinopathy). A universal problem for many of the studies analyzed was the absence of a "burning placebo" such as camphor.

“…This process is referred to as desensitization and has been exploited for its therapeutic potential. In effect, capsaicin-containing creams have been in clinical use for many years to relieve painful conditions such as diabetic neuropathy (33). Capsaicin is the key ingredient in the experimental drug Adlea (ALRGX-4975; Anesiva, Inc.), which is in phase III trials as a long-acting analgesic to treat postsurgical and osteoarthritis pain for weeks to months after a single injection to the site of pain (34).…”

“…Topical capsaicin has been suggested as an effective pain management adjunct for rheumatoid arthritis, osteoarthritis, neuralgias, diabetic neuropathy, and other conditions including neural dysfunction, inflammation, and painful or itching cutaneous disorders resulting from surgery, injury, or tumors (33). However, even in this arena, evidence for the effectiveness or safety of capsaicin use in pain relief is controversial.…”

“…Levels of neuropeptides have been shown to be increased in samples of synovial fluid from patients with rheumatoid arthritis (11), and the ligation of sensory nerves in experimental joint inflammation is associated with a beneficial effect (12), providing evidence for involvement of sensory nerve activation in arthritis. Indeed, continued topical treatment with capsaicin has been shown to be beneficial in patients with hand osteoarthritis (13), and capsaicin creams are marketed for the alleviation of pain in diseases including both osteoarthritis and rheumatoid arthritis (14). There is evidence from rodent studies that TRPV1 activation can influence pathophysiologic events in arthritis.…”

“…6 Some capsaicin containing creams are used for the treatment of painful conditions such as long-term neuropathic pain in cancer patients 7 and diabetic neuropathy. 8 The quantity of active compounds depends on several factors such as intra species variability, environmental conditions, geographical variations, harvesting and storage time and extraction methods. 9 Marker profiling and standardization of medicinal plants can help in finding an optimum concentration of bioactive compounds present in herbal drugs and thus ensures it quality.…”
