Review article: oesophageal dilation in adults with eosinophilic oesophagitis

Abstract: Alimentary Pharmacology and Therapeutics INTRODUCTIONEosinophilic oesophagitis (EoO) is a chronic inflammatory disorder of the oesophagus characterised by the proton pump inhibitor-refractory accumulation of eosinophils in the oesophageal epithelium [>15 intraepithelial eosinophils ⁄ high powered field (eos ⁄ hpf)] in combination with a range of symptoms including dysphagia, food impaction, chest pain, abdominal pain and vomiting. [1][2][3] The clinical manifestations of EoO vary with age. Nearly all adults co… Show more

“…The presented case reminds us that we should attempt to avoid food impaction in patients with eosinophilic esophagitis because endoscopic removal of the impacted food may cause perforation [2,3]. During endoscopy, preventative measures such as general anesthesia and the use of antiemetics may be required.…”

“…Although there was some initial concern that the rate of oesophageal perforation in these patients is higher, this has been disproved in observational studies. The rate of perforation remains <0.1%, 10 which is comparable to that of dilatation of any stricture. However, the incidence of mucosal tears and post-procedure chest pain is much higher, and should be emphasised to patients pre-procedure.…”

“…124 However recent studies including two systematic reviews have negated this concern and cumulative data on over 1000 dilatations has revealed a perforation rate of 0.3% only 125,126 which is similar to the rates associated with dilatation for other indications. Secondly all these perforations were managed conservatively and none required operative interventions.…”
