Moderne Geburtshilfe vollzieht sich zunehmend im Spannungsfeld zwischen biomedizinischer Technik und Psychoprophylaxe. Die Konfliktsituation hat sich in den letzten Jahren dadurch versch/irft, dag die organisatorische und technische Perfektion der Kreil3s~ile uns immer mehr dazu verffihrt, den Geburtsvorgang als einen autonomen, rein naturwissenschaftlich-mechanistischen Prozel3 zu verstehen, w~ihrend ffir die Schwangere selbst die Geburt nach wie vor ein auBerordentlich emotionell betontes Erlebnis darstellt. Es geh6rt zu unseren vornehmlichen Aufgaben als Geburtshelfer in diesem dualistischen Streit zu vermitteln. Unser Dilemma besteht darin, dag wir gleichzeitig und in einer Person dem medizinischen Fortschritt verpflichtet sind und ein psychomedizinisches Mandat unserer Patienten zu vertreten haben.Der weltweite, nicht nut in der Laienpresse vorgetragene Protest gegen eine in jeder Weise sterile und auch computerisierte Geburtshilfe entzfindete sich Anfang der 70er Jahre vor allem in England an dem Thema: Geburtseinleitung --elective induction. Die Protestwelle ffihrte zu einem vielbeachteten kritischen Artikel im Lancet unter dem Titel ,A time to be born" und auch zu einer Anfrage im Parlament, im House of Common ,,Birth induction". Im Lancet wird fiber Einleitungsfrequenzen von 50% und mehr berichtet und in der Parlamentsanfrage heigt es ,,Some hospitals were inducing birth so that maternity departments could close during the christmas period". Es wird die berechtigte Frage gestellt, ob es vertretbar sei, einen so pr~izis ausbalancierten physiologischen Vorgang wie den Geburtsbeginn, dessen Mechanismen im einzelnen nicht bekannt sind, ohne zwingenden medizinischen Grund in so groBem Umfange zu manipulieren. Die Vermutung wird ge~iugert, daft sachfremde, nicht medizinische Motivationen im Vordergrund stehen.Ohne Frage, bei so hohen Einleitungsfrequenzen werden geburtsphysiologische Normen auf den Kopf gestellt. Wenn als ein wesentliches Kriterium ffir Normalverhalten die Frequenz des in Frage stehenden Ereignisses in der Bev61kerung gilt, so sind spontaner Blasensprung und spontane Wehent~itigkeit in England weitgehend zur Abnormalit/it geworden.(2bereinstimmend wird daher in der britischen Presse eine kritische Bestandsaufnahme unserer Kenntnisse fiber den Geburtsbeginn gefordert, sowie ein Uberdenken der fibliehen Praxis zur Geburtseinleitung. Der Titel mag t~berraschen, die Bezeichnung ,,unzeitig" stammt von Prof. Willi Wolf, ehemals Direktor der Universit~ts-Frauenklinik Freiburg. Wolf hatte sich schon in seiner TObinger Zeit intensiv mit der Bedeutung der Fruchtblase ft~r den Geburtsvorgang befaBt und Probleme der Amnionruptur studiert. Er ver6ffentlichte seine Ergebnisse 1946 in einer viel beachteten -noch heute lesenswerten Monographie mit dem Titel: ,,Klinik des unzeitigen Blasensprungs". Dieses Buch galt aber Jahrzehnte als Standardwerk [10].Mit dem Begriff ,,unzeitig" umschreibt Wolf als Varianten des rechtzeitigen Blasensprungs, wobei er den vorzeitigen, den frt~hzeitigen und den von ihm so genannten verz6gerten Blasensprung unterschied. Von klinisch praktischer Bedeutung hielt er vor allem die Frfihformen der Amnionruptur (Abb. 1).Im anglo-amerikanischen Schrifttum gibt es die Differenzierung in vorund frt~hzeitigen Blasensprung nicht oder genauer gesagt, nicht mehr. Man spricht dort vom ,,premature ruptur of membranes", kurz PROM genannt und versteht darunter die Amnionruptur vor Wehenbeginn. Damit ist na-tt~rlich auch dokumentiert, dab dem Fruchtwasserabgang wfihrend der Er6ffnungsperiode nur wenig Bedeutung beigemessen wird. Das ist keineswegs das Ergebnis systematischer Untersuchungen, sondern wohl ,,Verdrfingungsstrategie" im Rahmen einer vorwiegend aktiv betriebenen Geburtshilfe mit der Empfehlung zur frfihen Amniotomie und einem hohen Anteil an elektiven Einleitungen.Auch bei uns waren die Anschauungen t~ber den Stellenwert des frahzeitigen Blasensprungs verschiedentlichem Wandel unter!egen, die Meinungen sind bis heute geteilt. W~hrend der sehr st~irmischen Entwicklungsphase der modernen Geburtshilfe und Perinatologie in den 60er Jahren ging der Respekt vor der Schutzfunktion des unverletzten unteren Eipols weitgehend verloren. Die Erhaltung der Fruchtblase pal3te auch nicht in das g~ngige geburtshilfliche Konzept. Man stand zu sehr unter dem Eindruck der m6glichen Vorteile einer elektiven Geburtsleitung durch frt~hzeitige Amniotomie. Auch die konsequente Ausnutzung der Intensivt~berwachungsverfahren lief3en die generelle Amnionruptur mit Geburtsbeginn notwendig erscheinen. Der frt~hzeitige Blasensprung war kein Thema mehr
