Review: Anal Intraepithelial Neoplasia in HIV-Infected Men Who Have Sex with Men: Is Screening and Treatment Justified?

Abstract: Anal squamous cell carcinoma (SCC) is the fourth most prevalent cancer in human immunodeficiency virus (HIV)-infected men who have sex with men (MSM). Human papillomavirus (HPV) has been detected in over 90% of anal carcinoma biopsy specimens from MSM, and is considered a necessary, but alone, insufficient factor for carcinogenesis. Anal intraepithelial neoplasia (AIN) may be precursive for SCC, and screening cytology with referral of persons with abnormality for high-resolution anoscopy-guided biopsy, and AIN… Show more

“…An important difference compared with the cervix is that excisions in the anal canal are associated with more adverse outcomes, which can have long‐term complications and affect anal sphincter functions . The decision to treat AIN2/AIN3 and the type of treatment depends on factors such as the size and location of the lesion.…”

“…48,49 An important difference compared with the cervix is that excisions in the anal canal are associated with more adverse outcomes, which can have long-term complications and affect anal sphincter functions. [50][51][52] The decision to treat AIN2/AIN3 and the type of treatment depends on factors such as the size and location of the lesion. Unlike cervical precancer, smaller lesions tend to be treated more often than larger lesions owing to the ease of treating these lesions with topical therapy or excision.…”

“…Anal SCC is one of the most prevalent cancers in HIV-infected patients and the fourth most common cancer in HIV-infected men who have sex with men (69). Approximately 90% of anal SCC in these patients has been associated with HPV infection (69). HPV is the most common sexually transmitted infection and is associated with anogenital warts, intraepithelial neoplasia, and anal carcinoma.…”

“…People living with HIV (PLHIV) are at increased risk of high-risk (HR)-HPV infection and persistence, (1) anal high-grade squamous intraepithelial lesions (HSIL+) or high-grade anal intraepithelial neoplasia (AIN2+) and incidence of anal cancer. (2,3) Anal cancer is the fourth most common cancer among men who have sex with men (MSM) living with HIV, (4) and evidence suggests higher rates of anal cancer among women living with HIV (WLHIV) and men who have sex with women (MSW) living with HIV compared to their HIV-negative counterparts. (3,(5)(6)(7) As antiretroviral therapy (ART) is scaled-up, longer survival times among PLHIV may be associated with an increase in the rates of anal and other cancers.…”
