a b s t r a c tIt has been 30 years since the Declaration of Alma Ata. During that time, primary care has been the central strategy for expanding health services in many low-and middle-income countries. The recent global calls to redouble support for primary care highlighted it as a pathway to reaching the health Millennium Development Goals. In this systematic review we described and assessed the contributions of major primary care initiatives implemented in low-and middle-income countries in the past 30 years to a broad range of health system goals. The scope of the programs reviewed was substantial, with several interventions implemented on a national scale. We found that the majority of primary care programs had multiple components from health service delivery to financing reform to building community demand for health care. Although given this integration and the variable quality of the available research it was difficult to attribute effects to the primary care component alone, we found that primary care-focused health initiatives in low-and middle-income countries have improved access to health care, including among the poor, at reasonably low cost. There is also evidence that primary care programs have reduced child mortality and, in some cases, wealth-based disparities in mortality. Lastly, primary care has proven to be an effective platform for health system strengthening in several countries. Future research should focus on understanding how to optimize the delivery of primary care to improve health and achieve other health system objectives (e.g., responsiveness, efficiency) and to what extent models of care can be exported to different settings.

Background: Near misses are defined as pregnant women with severe-life threatening conditions who nearly die but, with good care or good luck survive. Because near miss situation occurs more frequently than maternal death, more comprehensive and statistically reliable analysis could be conducted to assess the quality of maternal care and to develop evidence-based management protocols. The objectives of this study were to evaluate the determinants of near miss maternal mortality in a tertiary referral Government medical college hospital in Trivandrum, Kerala.Methods: Case control study from a defined delivery population with three randomly selected pregnant women as controls for every case. Study was conducted at Department of Obstetrics and Gynecology, Medical College Hospital Trivandrum, Kerala, India. Study duration was one year. Study population were patients admitted in Obstetrics and Gynecology Department, fulfilling the WHO criteria of near miss.Results: Of the primary determinant factors of near miss in this study, preeclampsia contributed the main role, followed by hemorrhage and eclampsia. During the antenatal period, preeclampsia was the major determinant followed by eclampsia. Hemorrhage was the major determinant in intra op /intra natal cases and next was eclampsia. Postpartum hemorrhage, eclampsia and preeclampsia were the major determinants in post-partum /post op cases. Among the associated indicators assessed, low socioeconomic status, anaemia, high body mass index, referral status, placenta praevia and caesarean section were statistically significant.Conclusions: Detailed analysis of near miss cases helps in identifying risk factors. It helps in formulating preventive strategies, and helps us in tackling the delays in referral process.

O objetivo deste trabalho foi realizar uma revisão integrativa na literatura sobre integralidade e atenção à saúde bucal. Pesquisaram-se publicações posteriores a 2004, oficiais (site do Ministério da Saúde) e trabalhos acadêmicos (Biblioteca Virtual em Saúde), com utilização de cinco descritores (‘integralidade saúde bucal’, ‘integralidade odontologia’, ‘integrality oral health’, ‘comprehensiveness oral health’, ‘integralidade’ – filtro ‘saúde bucal’). Após critérios de exclusão, foram lidas e analisadas, à luz dos sentidos da integralidade, 39 referências, dentre elas dois documentos oficiais e 37 trabalhos acadêmicos. Posturas profissionais diferenciadas, o modo como os serviços de saúde são organizados e o planejamento de políticas voltadas para a integralidade são conteúdos presentes nos trabalhos. A ideia de integralidade esteve ligada aos seus dispositivos (acolhimento, vínculo, responsabilização), à formação acadêmica, ao trabalho em equipe e à necessidade de diferentes níveis de assistência. Outros aspectos, como processo de trabalho, mecanismos de gestão e intersetorialidade, devem fazer parte de estudos futuros que envolvam a interface da integralidade com a saúde bucal.
