“…The extra-adrenal localization of these histologically benign tumors may be presacral (up to 50% of the extra-adrenal localization) (2), splenic, gastric, pulmonary, hepatic, retroperitoneal, testicular (1,7,8). Most of the time they are asymptomatic, unilateral (more frequent on the right side), but they can also be bilateral (9,10).…”

“…Adrenal myelolipomas are readily diagnosed on imaging because of their rich adipose tissue content. It is more difficult to diagnose a retroperitoneal extra‐adrenal myelolipoma as the imaging studies would be similar for a liposarcoma, renal angiomyolipoma, or retroperitoneal teratoma …”

“…Ultrasound shows an echogenic suprarenal mass, CT scan reveals hypodense, non-enhancing lesions with tissue density of fat. MRI demonstrates increased intensity in T1 and T2 weighted images which characteristically brighten with gadolinium contrast and diminish with phase cancellation and fat suppression [11]. Although specific, MRI was not done in this case because regardless of the findings, surgery was indicated given the large size and symptoms.…”
