Retroperitoneal ectopic pregnancies (REP) are extremely rare, and early diagnosis and treatment is very difficult. We completed an English literature search in MEDLINE through PubMed for articles on REP. We identified 14 articles (all case reports) but selected only 12 because of unavailable data in the other 2 articles. We also report the case of an REP which was misdiagnosed as ectopic choriocarcinoma. The 33-year-old woman was admitted via the outpatient department with a history of 54 days of amenorrhea and persistent elevated serum β-human chorionic gonadotropin (hCG) levels.The presumed diagnosis ectopic choriocarcinoma was made based on imaging findings. Single-drug chemotherapy with methotrexate (MTX; 20-mg intramuscular injection daily for 5 consecutive days) was administered. An upper abdominal mass was noticed by the patient and laparotomy was performed. A retroperitoneal pregnant lesion was found and removed successfully. The diagnosis, treatment and mechanisms of REP are discussed. We believe REP should be considered in patients with elevated serum β-hCG levels when the uterus and adnexa appear to be normal. Systemic administration of MTX in nonruptured REP before operation may prove to be helpful.

Ретроперитонеальная эк-топическая беременность (РЭБ) является крайне редким вариантом из всех эктопических беременно-стей, а эктопическая беременность двойной локали-зации (трубная и ретроперитонеальная) являет со-бой казуистический случай. Клиническое наблюдениеПациентка К., 37 лет, поступила в гинекологиче-ское отделение перинатального центра города Ниж-невартовска с жалобами на тянущие боли в пояс-ничной области слева. Из анамнеза известно о трех программах ЭКО: в одном наблюдении наступила трубная беременность справа, которая закончилась тубэктомией, остальные два переноса эмбрионов к беременности не привели. При четвертой попытке произведена подсадка двух эмбрионов (один -крио, другой в естественном цикле) in vitro. Отме-чался прирост хорионического гонадотропина человека-β (β-чХГ) в пределах нормы. Через 1 мес при контрольном УЗИ диагностированы эктопиче-ская беременность в левой маточной трубе и киста правого яичника. Выполнена лапароскопическая тубэктомия слева. Через 5 сут отмечалась боль в по-ясничной области слева, эритроцитурия, отсутствие снижения β-чХГ. Остальные лабораторные тесты были в пределах нормы. Ectopic pregnancy is an implantation of the fertilized ovum on a place except the endometrium. Most of the ectopic pregnancies are located at the fallopian tube. Only the 1.4% of ectopic pregnancies are abdominal pregnancies, of which only 15 cases of retroperitoneal pregnancies are reported all over the world. In this case, a 21-year-old woman presented with back pain and amenorrhrea for 5+2 weeks with no past history. During the laparoscopic operation, there was retroperitoneal hematoma which was located between right paracolic gutter and presacral area and there was no adnexal mass or free pelvic fl uid to be found. We stopped the operation and performed computed tomography angiography, in which small peripheral enhanced cystic lesion in pericaval space and large amount of hematoma in perirenal space were revealed. We performed explo-lapratomy, and eliminated the gestational sac which was located between the inferior vena cava and ureter. We report very rare case of retroperitoneal ectopic pregnancy with brief review of literature.
