Retinal neurovascular coupling in patients with glaucoma and ocular hypertension and its association with the level of glaucomatous damage

Abstract: General vessel response to flicker light was decreased in POAG patients, compared to normal controls and OHT patients. In contrast to significant correlation between the two contralateral eyes of the flicker response itself, only its borderline correlation to IOP was seen. There was no correlation to the level of damage, altogether indicating a systemic dysregulation phenomenon. “…192 Indeed, neurovascular coupling in the retina appears to be disturbed early in glaucoma. 190,193,194 In principle, restoration of neurovascular coupling may be an attractive approach for glaucoma, but the mechanisms that lead to its breakdown are poorly understood.…”

“…[11][12][13][14] Typically, the diameter of a vessel is assessed by fundus photographic techniques, which allow for the real time evaluation of retinal vessel diameter. A large number of studies have been performed to assess vessel diameter changes during stimulation with diffuse luminance flicker in healthy subjects under normal conditions 9,11 or during oxygen breathing, 15 as well as in patients with diabetes, 16,17 glaucoma, 18,19 or systemic hypertension. 19,20 However, the interpretation of the latter experiments is limited by the fact that only data about retinal vessel diameter, but not about volumetric blood flow, are available.…”
