Retinal nerve fiber layer defect patterns in primary angle-closure and open-angle glaucoma: A comparison using optical coherence tomography

Abstract: Participants with mild POAG had deeper and more localized RNFL defects than did participants with PACG. The pattern was similar in participants with moderate or advanced disease.

“…We found a very strong positive correlation between RPC and the MD (p ⩽ 0.001, r = 0.813) in ACC group, and between RPC and peripapillary RNFL (p ⩽ 0.001, r = 0.826). The relation between structure and function has long been studied by using OCT and VF, 17,18 but only few studies assessed the vascular perfusion and its relation to function in acute angle closure. 6,7,10 Zhang et al found a positive correlation between the peripapillary retinal vascular density and the RNFL (p < 0.0001).…”

“…APAC has traditionally been regarded as an acute occurrence but more recently has been found to be part of a chronic process [4,9,10]. This study suggests that progressive changes do occur following APAC, as detected using both HRT and GDx nerve fibre layer analysis [4,9,10].…”

“…This study suggests that progressive changes do occur following APAC, as detected using both HRT and GDx nerve fibre layer analysis [4,9,10]. At 18 months after the acute attack, 60% of patients showed progression in all HRT (rim volume, rim area, mean RNFL, C/D Ratio, and GPS) and GDx (TISNT Average, inferior average, superior average, TISNT SD, and NFI) parameters.…”

“…In the moderately advanced disease subgroups, RNFL defects were in sectors 1, 6, and 7. RNFL defect extended through almost all sectors in the advanced disease subgroups [23]. Analysis of the pattern of RNFL defects with SD-OCT imaging demonstrated that the most frequently RNFL defects have been at the inferotemporal meridian followed by the superotemporal meridian [24].…”
