Purpose: to describe peripapillary vascular changes using Optical Coherence Tomography Angiography (OCT-A) in patients with acute angle closure crisis (AACC) and primary-angle closure-suspects (PACS) in comparison to normal controls. Methods: This cross-sectional/case-control/non-randomized study was conducted at Cairo University Hospitals. It included 21 eyes following AACC, 21 eyes of PACS and 32 eyes of age-matched-controls. Participants underwent visual field (VF) examination, retinal nerve fiber layer (RNFL) assessment using spectral-domain-OCT (SD-OCT), and radial peripapillary capillary density (RPC%) using OCT-A. Results: There was a statistically significant difference in MD and PSD among the three groups ( p ⩽ 0.001). There was a significant difference in mean RNFL among the three groups ( p ⩽ 0.001), this decrease was still present when comparing the AACC group to controls p = 0.032. There was a significant decrease in the peripapillary RPC% in all groups p ⩽ 0.001. The correlation between structure, function and flow was studied for all groups. Peripapillary RPC% in AACC was positively correlated to MD and peripapillary RNFL ( p ⩽ 0.001). In PACS, RPC% was positively correlated to RNFL ( p = 0.012). In controls, RPC% was positively correlated to PSD and peripapillary RNFL ( p ⩽ 0.001). AUC was 0.8 for the MD, 0.56 for the RPC and 0.38 for the RNFL Conclusions: Peripapillary vessel density was lower in AACC eyes than in suspects and control eyes. OCT-A parameters could be a more sensitive marker than OCT parameters after an AACC attack as evident on ROC analysis. PACS remains a clinical diagnosis as we could not find any significant differences in OCT or OCT-A parameters between suspects and normal healthy controls.

Irreversible damage to the optic nerve can follow the rapid increase of IOP during an acute primary angle closure (APAC). Despite the development of advanced imaging technology, there is still a lack of good longitudinal studies assessing patients after an acute attack. The aim of this study was to assess the progression of patients using different objective optic nerve head and retinal imaging parameters following APAC.Twenty patients with a single attack of APAC, were retrospectively assessed in this study. Patients were assessed with the Heidelberg retinal tomography (HRT3) and scanning laser polarimeter (GDx-VCC) as well as Humphrey visual field (HVF) repeatedly up to eighteen months after the acute attack. Progression for each imaging modality was assessed with multiple parameters.All patients showed retinal nerve fibre layer (RNFL) and optic disc changes over time. At 18 months, 67% of patients showed progression in 4/5 GDx parameters, and 33% in all 5. HRT analysis similarly showed progression in 4/5 parameters in 70% of patients, and 30% in all 5.This study shows that structural progressive changes to RNFL and ONH occur following APAC confirming that APAC patients need long-term follow-up after the acute attack.

Purpose:To compare Primary Open Angle Glaucoma (POAG), Ocular Hypertension (OHT) and control groups by using Visual Field (VF) and Spectral Domain Optical Coherence Tomography (SD-OCT) and to investigate correlations between VF global indices and Retinal Nerve Fiber Layer (RNFL) thickness measurements.Methods: Forty patients with POAG, 55 patients with OHT, and 40 normal subjects were included in the study. All subjects were evaluated by standard automated perimetry and Cirrus HD-OCT.Results: RNFL global average thickness, average thicknesses in four quadrants and at 1 o'clock, 4 o'clock, 5 o'clock, 6 o'clock, 7 o'clock, 10 o'clock, 11 o'clock and 12 o'clock areas in POAG patients were significantly decreased compared with the OHT and the control groups. RNFL thicknesses at 2 o'clock, 8 o'clock and 9 o'clock areas in the POAG group were significantly lower than control subjects. There were statistically significant and a negative correlation between PSD and RNFL global average thickness, RNFL thicknesses in temporal quadrant and at 1 o'clock and 2 o'clock areas in the POAG group. According to the areas under the ROC curve, the parameter which has the best diagnostic ability was found as RNFL thickness in the superior quadrant. The sensitivity and specificity of Cirrus HD-OCT for RNFL thickness in superior quadrant were found 77% and 87%. Conclusion:The correlations between MD, PSD and RNFL thickness parameters could represent the consistency of functional and structural tests. RNFL measurement with SD-OCT could provide important information for detection of early stages of glaucoma.
