Retinacular Disorders of the Ankle and Foot

Abstract: The retinacula of the ankle are distinct structures defined as regions of localized thickening of the crural fascia covering the deep structures of the distal portion of the leg, ankle, and foot. Their role is to maintain the approximation of the tendons to the underlying bone. The retinacula of the ankle and foot contain the extensor retinaculum, the peroneal retinaculum, and the flexor retinaculum. Knowledge of the normal anatomy and imaging appearance of these structures is essential for the depiction of th… Show more

“…The ankle retinaculum is the focal thickening of the crural fascia that stabilizes the tendons to prevent bowstringing during muscle contractions [38]. The retinacula of the ankle and foot comprise the superior and inferior extensor retinacula (SER and IER) ( Figure 16A), the superior and inferior peroneal retinacula (SPR and IPR) ( Figure 16B), and the flexor retinaculum (FR) ( Figure 16C).…”

“…The SER is a transverse rectangular band proximal to the ankle joint. It attaches medially on the anterior crest of the tibia and medial malleolus and laterally on the lateral crest of the fibula and the lateral malleolus [38,39]. From medial to lateral, it covers the tibialis anterior, extensor hallucis longus, extensor digitorum longus, and peroneus tertius tendons.…”

“…At the malleolus, the two tendons are reflected on the anterior aspect of the external malleolus where they pass in an osteofibrous groove located anterior to the malleolus and posteriorly to the superior retinaculum [3,4,9,10] (Fig. 2b).…”

“…It is often slightly protruding and sometimes hypertrophied, thereby forming a kind of bone tunnel that surrounds part of one of the peroneal tendons. The peroneal brevis tendon and the peroneal longus tendon lie above and below the peroneal tubercle where they are stabilized by a second retinaculum (inferior retinaculum) which arises from the lateral aspect of the calcaneus and inserts on the apex of the tubercle [9,10].…”
