Reticulate pigmentation associated with scarring alopecia in an elderly woman: An unusual manifestation of lichen planus pigmentosus

Abstract: A 70-year-old woman presented with generalized reticulate pigmentation, scarring alopecia, and few discrete, violaceous plaques over the trunk and forearm. Dermoscopic evaluation of the reticulate plaque showed reticulate hyperpigmentation with multiple telangiectasias, and skin biopsy showed lichenoid interface dermatitis with marked pigment incontinence. Thus, a final diagnosis of poikiloderma due to lichen planus pigmentosus was considered.

“…Based on this hypothesis, we propose that the pigmentation in our patients likely began as a diffuse pattern that progressed over the years into the poikilodermatous form. The long duration of LPP lesions was a common finding in our patients and in the case reported by Sharma et al [1]. Prolonged use of topical corticosteroids and photo damage may also aggravate the poikiloderma.…”

“…Dear Editors, Poikilodermatous lichen planus pigmentosus (LPP) is a variant of LPP that has been previously described in one patient only [1]. It can be easily confused with other causes of poikiloderma and reticulate pigmentary disorders.…”

“…Reticulated hyperpigmentation limited to the scalp is rare. It has only been documented once in the literature as an extension of lichen planus pigmentosus (LPPi) [2]. Sharma et al described a case of diffuse LPPi with reticulated pigmentation associated with scarring alopecia; however, the type of scarring alopecia was not defined.…”
