Background: Currently, the perioperative care of fracture patients is compromised due to the outbreak of COVID-19 in China and the world. This study aims to assess the clinical features of fracture patients at our hospital during the COVID-19 outbreak and formulate the medical steps to ensure the effective treatment of fracture patients with minimal risk of infection to healthcare workers.Methods: One hundred twelve patients with different fractures that were admitted to the orthopedics department of our hospital from January 24 to March 9 in 2020 were reviewed. Data including age, gender, injury location, admission time, operation time, discharge time were compared with fracture patients from the same period in 2019Results: Compared to the same period in 2019, there is a 42% decrease in the number of fracture patients in 2020. Specifically, the incidences of forearm, thigh, hand, and foot fractures have increased during the COVID-19 outbreak, while other parts are less affected. The time from injury to hospitalization, the surgery wait time and time of discharge after surgery for patients with hip fractures were 2.9±7.1, 2.0±1.7 and 7.7± 4.0 days respectively in 2019, which changed to 2.0±5.0, 4.5±4.0 and 10.6±4.2 days in 2020. Following the orthopedic treatment regimen followed at our hospital, all patients had non-life-threatening limb fractures.Six patients were operated in a negative pressure room, and emergency screening was completed for six patients. No patients were positive for COVID-19, and all were discharged safely without infection or other serious complications.Conclusions: Hip fractures are highly prevalent during this epidemic. However, mandatory screening delays surgery by more than 48 hours. The orthopedic department should prioritize screening of emergency patients to minimize the risk of infection among other patients and medical personnel.

Background Family members in many countries often share caregiving responsibilities for an older relative recovering from an injury. However, few studies have examined strategies employed when multiple family members provide care for an older relative recovering from hip‐fracture surgery. Objective This study aimed to understand family group caregiving strategies when two or more family members provide caregiving for an older relative recovering from hip‐fracture surgery. Methods This study used a grounded theory design. Semistructured interviews were conducted over 1 year with 13 Taiwanese family caregivers from five families. Caregivers shared caregiving responsibilities for an older relative (62–92 years of age) recovering from hip‐fracture surgery. Transcribed interviews were analysed using open, axial and selective coding. Results The core category describing caregiving among family members was ‘Preventive Group Management: strategies for family group caregiving’. Three strategies were employed: explicit division of labour (two stem/patriarchal families and one older two‐generation/democratic family); disconnected caregiving (one nuclear/noncommunicative family) and patriarchal caregiving (one extended/traditional Chinese family). Strategies reflected family type, structure, cultural values, communication patterns and available outside support. Components of family group caregiving involved family type's division of labour, approaches to caregiving and implementation challenges and allowed family caregivers to maximise safety and stability and prevent harmful events during their relative's recovery from surgery. Conclusions There was no one‐size‐fits‐all approach for the strategies of family group caregiving. Components of Preventive Group Management varied with family type, cultural values, communication patterns and available outside support. Healthcare professionals should be sensitive to the dynamics of family caregivers. Implications for Practice Enhance group management for family caregivers by developing interventions to optimize collaboration, thereby better addressing the needs of older adults recovering from hip fracture surgery.

Businesses carried out worldwide have been influenced by the phenomenon of Globalization 4.0. has allowed us to make various changes in all aspects and activities of the human being in different countries where an essential interdependence of markets was reflected. However, they allow reflection on all economic activities. In the particular case of small and medium-sized enterprises (SMEs), this phenomenon creates an uncertain environment and opens the opportunity to develop a competitive environment and identify challenges and opportunities. Many elements are considered incident factors caused by Globalization 4.0 that affect Mexican SMEs. This work shows what factors affect the economic aspects and how the cultural factor is significant; the challenge of teamwork, cooperation, unity, and management of existing resources is also considered. Although Mexico has an influential business culture, it is not implemented in these companies; some seem unaware of a situation limiting their participation in conducting international business and improving their global competitiveness. This article shows the results of qualitative research based on the grounded theory proposed by Glaser and Strauss in 1967; the results obtained show how it is necessary to develop cultural aspects so that SMEs can achieve development and permanence over time. Globalization 4.0 is a phenomenon that affects international companies and represents actions and strategies to have an advantage over other companies, but what happens with SMEs is different; these types of companies do not have economic resources, and globalization 4.0 becomes a disadvantage. In our study, 80 % of SMEs do not have a business culture and are not prepared for globalization 4.0, and we also found that time is wasted on bad practices in 35 % of Mexican SMEs; training was lacking in 32 % of educational cases.
