Objective. To evaluate the efficacy of immunological tests for early detection of tuberculosis infection in children in the Yaroslavl Oblast (Russia).Material and methods. Medical records of 354 children and adolescents examined by ELISPOT (Enzyme-Linked ImmunoSpot) assay (T-SPOT.TB) in the Yaroslavl Oblast in 2020–2022 were studied. Four groups of children were distinguished: group 1 included children with active tuberculosis (n = 3); group 2 included children under 7 with altered tuberculin sensitivity (conversion of the tuberculin test) or children over 8 first-time tested positive for RTA, who were under the supervision of a phthisiatrician in the VI A group of dispensary registration (n = 52); group 3 included children with medical exemptions from screening immunodiagnostics (n = 49); and group 4 included children with refusals of their parents or legal representatives from skin tests (n = 250).Results. The sensitivity of both RTA and T-SPOT.TB tests achieved 100%, with the concordance level of 100%. When assessing discordant results (positive result for RTA and negative for T-SPOT.TB) in the VI A dispensary registration group, the majority of children were found to have an aggravated allergic anamnesis and somatic pathology. The maximum number of children examined by the laboratory method included those whose parents or legal representatives refused from skin test administration (70.6%).Conclusions. T-SPOT.TB is an optimal method for early detection of latent TB infection and TB in children at risk, including those with medical contraindications for skin tests or whose parents refused from screening immunodiagnostics.

Background: After the breakup of the Soviet Union, the annual incidence of tuberculosis (TB) in children 15-17 years of age increased in the Russian Federation from 16 per 100 000 population in 1992 to 37 per 100 000 in 2009, and new control measures were implemented. Methods: Children were screened annually for TB exposure with a tuberculin skin test (TST) at age 1-8 years. If positive, they were investigated for active TB. If no active TB was found, they were treated with isoniazid for 4-6 months; they then underwent 6-monthly skin tests (which included two recombinant proteins) until negative and annual skin tests thereafter. From the age of 8 years, the yearly follow-up was performed using the skin test that included two recombinant proteins, either until they became negative, developed active TB, or turned 18 years. Results: The annual incidence of TB in Russian children decreased from 19.1 per 100 000 population in 2001 to 8.3 per 100 000 population in 2018. Conclusions: Annual screening for TB exposure with treatment for latent or active TB has reduced the annual incidence of TB in Russian children aged 15-17 years to 1992 levels.

For the immunodiagnosis of tuberculosis infection, in addition to standard skin tests (Mantoux test with 2 TU PPD-L and Diaskintest), in vitro tests are currently used, one of which is the T-spot.TB test. Purpose of research. Evaluate the T-SPOT test as a method of early detection of tuberculosis infection in children and adolescents and its role as an additional method in solving the problem of the need for chemoprophylaxis. Materials and methods. The analysis of the results of T-SPOT.TB in 794 children aged 2 to 17 years inclusive in the period from 2016 to 2019 was carried out. Two groups were allocated: 1 group (n = 596)-children who underwent T-SPOT.TB as the main methodology; Group 2-198 children with positive skin test results for a recombinant tuberculosis allergen after screening. The results of T-SPOT.TB were evaluated taking into account the data of previous immunodiagnostics and associated pathology. Results. It is established that T-SPOT.TB. can be used as an independent method in case of rejection of skin tests. The level of latent tuberculosis infection is higher among children with concomitant pathology according to the results of T-SPOT.TB twice. Conclusion. T-SPOT.TB can be an alternative method for diagnosing tuberculosis infection during screening. In children with concomitant pathology of the T-SPOT.TB can serve as the leading method of tuberculosis immunodiagnosis. A positive T-SPOT.TB can serve as an additional method for deciding on the appointment of preventive chemotherapy.
