Background: Chronic suppurative otitis media (CSOM) is a long-standing infection of middle ear cleft, characterized by ear discharge and permanent perforation of tympanic membrane. Surgical treatment of CSOM is still controversial. Recently many studies have been undertaken to evaluate the prognostic impact of various factors on the outcomes of tympanoplasty. Nonetheless, only few studies have assessed the clinical preoperative assessment method that allows for tympanoplasty outcome to be predicted. Objective: To examine the role of the prognostic factors and middle ear risk index on the success of tympanoplasty.Patients and methods: The medical records of 100 patients who underwent type 1 tympanoplasty between 2018 and 2020 were prospectively reviewed. Prognostic factors such as age, sex, presence of systemic diseases, location and size of perforation, duration of dry period, presence of myringosclerosis, presence of septal and conchal pathology, operation type, and status of the opposite ear were evaluated. Middle ear risk index (MERI) was calculated and its correlation with the rate of success of surgery was evaluated. Results: It was noticed that experience of surgeon more than 5 years, absence of comorbidities and low middle ear risk score were significant predictors for success of tympanoplasty type 1 (P < 0.001, P < 0.001, P < 0.001 respectively). Conclusion:We found that younger age, female gender, no comorbidities, and higher surgeon experience were associated with better outcome. In addition, patients with mild MERI score showed higher rate of success for tympanoplasty. The surgeon experience and MERI were independent predictors of successful procedure.

Reducing Surgical Site Infections During Otolaryngology Surgical Missions

Objectives:To determine what measures an otolaryngology-head and neck surgery team might adopt to decrease the incidence of surgical site infection (SSI) on a short-term surgical mission. Despite concerns about safety and efficacy, shortterm surgical missions remain the predominant structure for humanitarian surgical care in low-and middle-income countries (LMIC). Hospitals in high-income countries strive to improve surgical outcomes through implementation of World Health Organization (WHO) safe surgery guidelines. Reduction of SSI risk is a key part of this effort.Methods: Literature review and practical experience.Results: WHO recommendations for reducing SSI are based largely on research done in the North America and Europe. LMIC populations are younger; comorbidities are fewer; infectious disease and trauma are prevalent; and delays in access to care are common. SSI are much more frequent in resource-limited settings. Recommendations regarding preoperative assessment, operating room environment, instrument sterilization, surgical antibiotic prophylaxis, surgical site preparation, gloving, draping, and postsurgical care are reviewed in the context of a surgical mission at a typical LMIC government hospital.Conclusion: Many of the WHO guidelines on reduction of SSI are logical and applicable to the short-term surgical missions; others may need to be modified. Careful prospective data collection and clinical trials are needed to learn which interventions are valid and which should be changed.

BackgroundMillions of people across the world suffer from disabling hearing loss. Appropriate interventions lead to improved speech and language skills, educational advancement, and improved social integration. A major limitation to improving care is identifying those with disabling hearing loss in low-resource countries.ObjectivesThis review article summarises information on currently available hearing screening platforms and technology available from published reports and the authors’ personal experiences of hearing loss identification in low-resource areas of the world. The paper reviews the scope and capabilities of portable hearing screening platforms, including the pros and cons of each technology and how they have been utilised in low-resource environments.ConclusionPortable hearing screening tools are readily available to assess hearing loss in low-resource areas. Each technology has advantages and limitations that should be considered when identifying the optimal methods to assess needs in each country.
