Results of macular pucker surgery: 1- and 5-year follow-up

Abstract: Although macular pucker surgery resulted in an increase in vision and a decrease of metamorphopsia in 83% of patients after 1 year, we assume, based on the biased patient sample we questioned after 5 years, that subjective assessment would be positive in only a small majority of the patients.

“…No differences between the responders and non-responders could be identified with regard to age, pre-and postoperative BCVA, or mode of treatment. Thus, we could not find any difference between non-responders and responders as had Bouwens et al (2008), who concluded that nonresponders had inferior benefit from the operation than responders. In our study 88% of the patients indicated that they would choose surgery again if they knew the result beforehand, compared with 70% in the study of Bouwens et al (2008).…”

“…In the review of the literature on predictive factors for postoperative VA by Scheerlinck et al (2015) they found that the only consistently associated factor was preoperative VA and the severity of metamorphopsia was possibly associated with postoperative VA. Numerous studies have attempted to grade the metamorphopsia (Bouwens & Van Meurs 2003;Bouwens et al 2008) by grading the area or degree of the metamorphopsia. Arndt et al (2007) performed quantitative analyses of the degree of metamorphopsia preand postoperatively.…”

“…Lower preoperative VA, a longer duration of symptoms, the presence of cystoid macular edema (CME), greater membrane thickness, and older age have all been suggested as adverse prognostic factors [2,10,12,13]. The effect of additional ocular morbidity other than ERM in eyes undergoing surgery has not been extensively evaluated.…”

“…Incidence rises with age from 6% of the population above 50 years of age to 20% of people older than 75 years, and it is bilateral in 10–20% of cases [2,3]. ERM cause decreased visual acuity (VA) and metamorphopsia [4,5] that seldom improve without surgical intervention.…”
