Resting Blood Flow in the Skin: Does it Exist, and What is the Influence of Temperature, Aging, and Diabetes?

Abstract: Measurement of resting blood flow to the skin and other organs is an important indicator of health and disease and a way to assess the reaction to various stimuli and pharmaceutical interventions. However, unlike plasma ions such as sodium or potassium, it is difficult to determine what the proper value for resting blood flow really is. Part of the problem is in the measurement of blood flow; various techniques yield very different measures of skin blood flow even in the same area. Even if there were common te… Show more

“…There are several methods for measuring skin blood flow, including venous occlusion plethysmography, laser Doppler, ultrasound, thermostrom, photoelectric plethysmography, impedance, and radioactive isotopes [37,38]. Venous occlusion plethysmography (VOP) measures the blood flow by measuring the change in the volume of an organ or limb, usually the forearm [39].…”

“…However, because the VOP renders 6 www.escholarship.org/uc/item/37d3q23w hand ischemia, it cannot gather blood flow data for more than 13 minutes, and it cannot be used during exercise or any other type of movement. Doppler ultrasound uses the Doppler technique to measure the speed of blood flow velocity with ultrasound and finds conduit vessel diameters to determine blood flow [37]. The ultrasound head must always be perpendicular to the artery and any movement results in a large error.…”

“…However, it has to be noted here that, for the current assessment, a series of assumptions were made which underlie the high degree of uncertainty which accompanies this approach. For example, our efforts might be hampered by 1) lack of an appropriate physiologically based skin model, 2) possible permeability limitation of intrinsic clearance in skin explants, 3) high inherent variability of skin blood flow (Petrofsky, 2012), 4) uncertainty of the drug's unbound fraction in skin, and 5) almost complete lack of suitable in vivo skin metabolism data for model validation. Besides that, current assessment exclusively focuses on the systemic elimination effect of skin metabolism.…”

“…While the subjects with diabetes had significantly less blood flow perfusing the skin during this first minute, this is not surprising since previous studies show impairment in the ability of the skin to response to heat in people with diabetes due to endothelial cell dysfunction [10,[37][38][39][40]. The blood flow in the first minute after application of the heat in either group of subjects comparing the pre and post vitamin D administration data shows no effect of vitamin D. The initial blood flow increase with heat is mediated by CGRP and SP [29,30] was unaltered by acute administration of vitamin D. But the sustained response of blood flow to heat is mediated largely by NO [30,31].…”
