Background/Aim: There is a need of investigation of high frequency of the cases of ABO discrepancy revealed by forward and reverse typing. The aim of the study was to reveal optimal methods for detection of weak subgroups of group A and B antigens on erythrocytes. Methods: Sixteen persons with A blood group and anti-B antibody adsorbing ability (weak Bel, Bm and B3 antigens), 5 persons with B blood group and anti-A antibody adsorbing ability (weak Ax antigen) and 2 persons with O blood group and anti-B antibody adsorbing ability (weak Bel antigen) were investigated as the cases of type II-III of ABO discrepancy and weak B antigen. Liquid chromatography, agglutination, inhibition of agglutination and adsorption reactions at 4 °C and 37 °C with prolonged incubation (12 hours) were used in the study. Results: The persons with A blood group revealed by forward typing at 4 °C and O blood group detected by reverse typing at 37 °C showed the ability of the erythrocytes to adsorb anti-B antibody. The persons with B blood group revealed by forward typing with presence of anti-B antibodies reactive at 37 °C showed the ability of erythrocytes to adsorb anti-A antibody. Prolonged incubation at reverse typing, adsorption-elution tests, inhibition of haemagglutination and liquid chromatography were used to define blood group specificity. Conclusion: Prolonged incubation during reverse typing at 4 °C and 37 °C, adsorption at 4 °C, heat elution and liquid chromatography might help to detect blood group type in cases of type II-III of ABO discrepancy.

Objective: To determine the frequency of ABO discrepancies in pediatric patients of lymphoma and solid organ tumors and to categorize these discrepancies and their resolution. Study Design: Cross-sectional study. Setting: Department of Hematology & Transfusion Medicine, The University of Child Health Sciences & The Children’s Hospital, Lahore. Period: November 2020 to September 2021. Material & Methods: ABO blood group discrepancies were assessed by tube method of blood grouping, using antisera A, B, AB & D for forward grouping and A, B, and O cells for reverse grouping. Auto control was also run. The resolution techniques were used accordingly. The collected data was checked for its completeness, consistency and accuracy before analysis which was done on SPSS version 26. Results: In this study, a total of 105 subjects were included with mean age of 5.64 ±2.1 years. Among them 72(68.6%) were male and 33(31.4%) were female. Out of total samples processed, three (2.9%) discrepancies found, 1 in female and 2 in male patients. There were two cases of Group I ABO discrepancies, one of which was resolved by elution, antibody screening and identification and other by incubation at 37oC and correlation with previous transfusion history. One case of Group II ABO discrepancy which was resolved by incubation at 40C for 30 minutes. Conclusion: This shows that ABO discrepancies occur in pediatric patients of lymphoma and solid organ tumors. So, the interpretation of forward and reverse ABO blood grouping, identification and resolution of ABO discrepancies in these patients should be done very carefully to avoid any transfusion related adverse reactions.

Introduction: Mixed agglutination reaction is used for revealing A,B and H blood group antigens on the cells. The aim of the study was to visualize the antibody binding on weak blood group antigens of leukocytes in order to improve the quality of blood typing. Material and methods: The antibodies were contacted with group A and B leukocytes at 4 C. Mixed agglutination reaction was used to reveal group antigens on leukocytes using test erythrocytes. Results: The heated anti-A,B serum after adsorption on group A leukocytes agglutinated group A erythrocytes. Anti-A,B heated plasma after the contact with group B leukocytes agglutinated group B erythrocytes. Anti-A,B serum after adsorption on group B(A+) leukocytes agglutinated group B (A+) erythrocytes. Anti-A,B serum after adsorption on group A(B+) leukocytes agglutinated group B erythrocytes. Conclusions: MAR allowed to reveal both agglutinogenic and weak adsorbing blood group antigens, that helps to perform accurate blood group typing, especially for transplantation goals. The study revealed, the detection of adsorbing antigens of leukocytes should be performed at 4 C.
