Summary.We investigated a patient with a long-standing IgGk monoclonal gammopathy who developed severe haemorrhagic complications. At IgG concentrations of ϳ50 g/l the patient had severe bleeding associated with prolongation of the thrombin time, activated partial thromboplastin time, and reptilase time. Plasmapheresis resulted in improvement in the thrombin time and resolution of bleeding. Depletion of the IgG by absorption of plasma with protein G-Sepharose in vitro resulted in normalization of the thrombin time and reptilase time. The purified IgG bound to immobilized thrombin and immunoprecipitated human a-, b-and g-thrombin but not prothrombin, other vitamin K-dependent coagulation factors, or fibrinogen. Purified IgG at concentrations >1 × 10 ¹2 g/l decreased (ϳ50%) the rate of hydrolysis of a chromogenic substrate by thrombin. Addition of purified IgG to normal pooled plasma at concentrations >1 × 10 ¹2 g/l prolonged the thrombin time and activated partial thromboplastin time, but the reptilase time was prolonged only at IgG concentrations >1 g/l. This finding suggests that at low concentrations the IgG produces a specific antithrombin effect, but at higher concentrations it also affects fibrin polymerization; the combination of these effects probably produced clinical bleeding. This is the first report of a monoclonal antithrombin antibody associated with bleeding in a patient with multiple myeloma.

RESUMO:As serpentes do gênero Bothrops são responsáveis por cerca de 90% dos casos de acidentes ofídicos em seres humanos na América do Sul, sendo que deste valor, 89% dos casos ocorrem no Brasil, isso ocorre devido ao fato de que o Brasil possui as características necessárias para o habitat das serpentes botrópicas. O acidente botrópico apresenta como características as marcas das presas da serpente no local da picada, edema, sangramento local e até necrose; sendo assim o veneno possui ações proteolíticas, vasculotóxicas e coagulantes. Para o diagnóstico do acidente botrópico deve-se levar em consideração o histórico do paciente, achados no exame físico e nos exames laboratoriais, que sugiram o acidente. Sempre que houver suspeita de acidente botrópico deve ser avaliado o tempo de coagulação sanguínea (TC), que geralmente encontra-se de aumentado (maior que 10 minutos) a incoagulável (maior que 30 minutos). O tratamento baseia-se na soroterapia devendo ser realizado o mais rápido possível, podendo ser utilizado o soro antibotrópico ou soro polivalente. Considerando a importância dos acidentes botrópicos no nosso país, objetivou-se com esta revisão bibliográfica abordar o acidente botrópico, tanto pelas suas propriedades tóxicas quanto farmacológicas, bem como as medidas terapêuticas na condição deste tipo de acidente.Palavras-chave: Acidente ofídico. ACCIDENT CAUSED BY Bothrops IN SMALL ANIMALS -REVIEWAbstract: The Bothrops snakes are responsible for about 90% of cases of snake bites in humans in South America, and of this amount, 89% of cases occur in Brazil, this is due to the fact that Brazil has the features needed for the habitat of snakes bothrops, in addition to being distributed throughout the country. The bothropic accident has features like the marks of fangs of the snake bite site, edema, local bleeding and necrosis; therefore the poison has proteolytic actions, vasculotóxicas and coagulants. To diagnose bothropic accident should take into consideration the history of the animal, physical examination and laboratory tests, which usually show mild anemia, leukocytosis by neutrophilia and thrombocytopenia. Whenever there is suspicion bothropic accident should be rated the blood clotting time (CT) which usually is currently increased (greater than 10 minutes) incoagulable (greater than 30 minutes) .The treatment is based on serum therapy should be performed ASAP, may be used or polyvalent antivenom serum, which is provided in a dose that neutralizes at least 100 milligrams of Bothrops venom. Considering the importance of Bothrops accidents in our country, this literature review aimed to address the bothropic accident, both for its toxic properties as pharmacological.
