CSF β-Amyloid 1-42 Predicts Progression to Cognitive Impairment in Newly Diagnosed Parkinson Disease

BACKGROUND Low CSF β-amyloid 1-42 has been associated with cognitive decline in advanced Parkinson’s Disease; data from a single cohort suggest β-amyloid 1-42 may be an early marker of cognitive impairment. DESIGN/METHODS Newly diagnosed Parkinson’s participants (mean duration: 6.9 months) in the Parkinson’s Progression Markers Initiative (n=341) were assessed at baseline (untreated state) and followed for two years. CSF β-amyloid 1-42, α-synuclein, total tau, and tau phosphorylated at threonine 181 were collected at baseline. Participants were classified as having CI if scores on two of six cognitive tests were 1.5 standard deviations below the standardized mean based on published norms in healthy controls. Multivariable regression analyses was used to determine the association between baseline CSF markers with cognitive impairment, defined by neuropsychological testing performance at two-year follow-up. RESULTS 55 participants (16.1%) had CI at baseline and were not included in further analyses. 37 of the 286 participants without CI at baseline (12.9%) developed CI at two years. Participants with CI at two years had significantly lower mean baseline CSF β-amyloid 1-42 levels than non-CI participants (343.8 vs. 380.4pg/mL, p<0.01); no significant difference was seen for α-synuclein, T-tau, or P-tau 181. In a regression model of 286 participants without baseline CI adjusted for age, gender, disease duration, education, motor severity, and depression status, lower baseline β-amyloid 1-42 levels were associated with higher odds of CI at two years. (OR10pg/mL=1.04, 95%CI: 1.01–1.08, p<0.05). CONCLUSION CSF β-amyloid 1-42 level at disease onset is an independent predictor of cognitive impairment in early Parkinson’s Disease.

Research has shown that children of depressed mothers are at risk for problems in a variety of developmental domains; however, little is known about the effects of maternal depression on children’s emerging understanding of false beliefs. In this study, three false belief tasks were administered to five-year-old children whose mothers had either met criteria for major depressive disorder within the first 20 months of the child’s life (n = 91) or had never been depressed (n = 50). Significant difficulties in performance were found among the children of depressed mothers, especially those whose mothers had experienced early and recent recurrent depressive disorder. Regardless of diagnostic status, children whose mothers exhibited negativity during problem-solving tasks administered at an earlier developmental period also were less likely to demonstrate false belief understanding. These effects remained even after child verbal ability was controlled.

______________________________________________________________________ RESUMO: Recentemente, tem sido realçada a importância do distress psicológico (DP) no impacto na saúde, qualidade de vida e psicopatologia dos indivíduos. A presente investigação tem como principal objetivo contribuir para a adaptação do Symptom Questionnaire (SQ-48) para a população portuguesa, um questionário de avaliação do DP que inclui constructos psicológicos negativos e positivos avaliados segundo nove subescalas: Humor, Ansiedade, Somatização, Agorafobia, Agressão, Problemas Cognitivos, Fobia Social, Funcionamento no Trabalho e Vitalidade/Otimismo. Foi utilizada uma amostra de 758 estudantes académicos de enfermagem (81,7% do sexo feminino) com idades compreendidas entre os 17 e os 43 anos (M=20,7; DP=2,8). Foram conduzidas Análises Fatoriais Confirmatórias (AFC) para a análise da estrutura fatorial do SQ-48 e procedeu-se ao estudo da consistência interna e das validades. Os resultados da AFC evidenciam índices de ajustamento satisfatórios. A consistência interna foi avaliada através do alfa de Cronbach e foram obtidos resultados satisfatórios para as nove subescalas do SQ-48, com oscilações entre os 0,626 e os 0,870 valores. No geral, as subescalas do SQ-48 associaram-se significativamente entre si e os resultados sustentam boa validade convergente. O instrumento final e as suas características psicométricas apresentam algumas diferenças com o instrumento do estudo original e os resultados obtidos levantam algumas questões que são discutidas de forma sustentada. Os resultados validam o SQ-48 como um instrumento de avaliação consistente e multidimensional do distress psicológico, com itens devidamente adequados à medição dos constructos subjacentes. São avançadas considerações para investigações futuras.
