> Le traitement de l'infection chronique par le virus de l'hépatite C (VHC) a singulièrement progressé ces deux dernières décennies. Depuis 15 ans environ, la combinaison de l'interféron  pégylé et de la ribavirine permet d'obtenir un taux de réponse virologique prolongée, assimilable à une guérison, chez 45 % des patients infectés par un génotype 1, 65 % de ceux infectés par un génotype 4, 70 % de ceux infectés par un génotype 3 et environ 85 % de ceux infectés par un génotype 2. Une meilleure compréhension du cycle réplicatif du VHC a permis le développement d'antiviraux directs spécifiques du virus ciblant les protéines virales (la protéase NS [non structural] 3/4A, la polymérase NS5B avec des inhibiteurs nucléosidiques et non nucléosidiques, la protéine multifonctionnelle NS5A du complexe de replication). De nombreux résultats ont montré que des combinaisons d'inhibiteurs de première génération, notamment les inhibiteurs de protéases, en association avec l'interféron pégylé et la ribavirine, avaient une efficacité antivirale élevée (75 % de guérison des patients infectés par un VHC de génotype 1) avec une tolérance difficile. Cette première étape majeure a été rapidement suivie de l'utilisation d'une combinaison des antiviraux directs de deuxième génération avec l'interféron pégylé et la ribavirine, permettant d'augmenter sensiblement les taux de guérison (de 75 à 90 %) avec une réduction des durées de traitement et du nombre de comprimés administrés. La prochaine étape sera la combinaison des antiviraux directs, qui deviendra le standard de traitement en 2015. La plupart des études ont inclus des patients « faciles à traiter », en petit nombre, avec des résultats d'efficacité et de tolérance remarquables. Cependant, les résultats obtenus avec les « populations difficiles à traiter » (sujets en échec de traitements antérieurs, notamment par les inhibiteurs de protéases, sujets cirrhotiques, sujets transplantés hépatiques ou rénaux, et sujets infectés par le VIH [virus de l'immunodéficience humaine]) restent à confirmer et sont impatiemment attendus. < . La véritable révolution thérapeutique à laquelle on assiste aujourd'hui est probablement unique, à plus d'un titre. L'infection par le VHC est la seule infection chronique dont on guérit. La rapidité de développement de nouveaux traitements, constamment plus efficaces et mieux tolérés, est sans doute sans comparaison dans l'histoire pharmaceutique. Les capacités de remodelage et de régé-nération hépatiques, permettent potentiellement, avec la guérison virale, la réparation des lésions, notamment de fibrose, générées lors de l'infection. S'ouvrent ainsi de nouveaux défis pour un dépistage plus large, un accès facilité aux traitements les plus efficaces, et une réflexion sociétale sur le rapport coût/efficacité des stratégies diagnostiques et thérapeutiques. Background: Detection of hepatitis C virus specific antibodies is the initial step in chronic HCV diagnosis. HCV NS4B is among the most immunogenic HCV antigens and has been widely used in commercial Enzyme Immunoassays (EIA). Additionally, NS4B, a key protein in the virus replication, can be an alternative target for antiviral therapy. Objectives: Development of a new method for high-level expression and purification of NS4B coding region was the aim of the report. Materials and Methods: Viral RNA was purified from the serum of an HCV positive patient and NS4B coding region was amplified using nested RT-PCR. PCR products were cloned into pET102/D-TOPO expression vector and transformed into E. coli BL21. Induction was performed by adding 1 mM isopropyl-β-D-thiogalactopyranoside (IPTG) to the culture medium. Immunoreactivity of the purified recombinant proteins was evaluated by immunoblotting and indirect enzymelinked immunosorbent assay (ELISA). Results: The recombinant NS4B protein was expressed and its immunoreactivity was confirmed by ELISA and western blotting. Conclusions: The directional TOPO cloning provides an efficient and easy platform for heterologous expression of immunoreactive HCV NS4B.

The establishment of a robust hepatitis C virus (HCV) cell culture system and subsequent detailed characterization of the HCV life cycle was a key step toward the identification of putative antiviral targets and respective antiviral drugs. 1,2 These include direct-acting antiviral agents (DAAs) such as NS3/4A protease inhibitors, NS5A inhibitors, nucleos(t)ide and non-nucleoside polymerase inhibitors, as well as hosttargeting agents (HTAs) directed against cellular factors involved in viral entry or replication. 3 Trials investigating different drug classes with and without pegylated interferon (PegIFN) and/or ribavirin have rapidly evolved, yielding highly promising sustained virologic response (SVR) rates. 4,5 In regard to the enormous public health impact of HCV infection with an estimated 184 million anti-HCV positive persons worldwide, the recent advances in HCV drug development, together with implementation of improved HCV screening programs, have the potential to substantially reduce the burden of HCV-associated advanced liver disease including hepatocellular carcinoma. 6-9The first-in-class NS3/4A inhibitors telaprevir and boceprevir were approved in 2011. Addition of these drugs to the PegIFN/ribavirin backbone has considerably improved response rates in HCV therapy naïve and experienced patients compared with PegIFN/ribavirin alone. [10][11][12][13] The poor side-effect profile of these IFN-based therapies, however, limits the wide clinical applicability. Proof of the principle that viral clearance can be achieved with an IFN-free DAA combination has fast-tracked design and the conduct of clinical trials investigating IFN-free DAA combinations. 14 Different HCV genotypes and multiple subtypes result in a genotype and subtype-dependent efficacy of many available DAAs. 15,16 Furthermore, viral clearance is challenged by the presence and selection of resistance-associated variants (RAVs). 17,18 Thus, essential prerequisites for DAAs in clinical development comprise a high antiviral efficacy, a broad and at best pan-genotypic activity, as well as a high barrier to resistance.
