Repeated resections for liver metastasis from primary adrenocortical carcinoma: A case report

Abstract: HighlightsAdrenal cortical carcinoma (ACC) is rare, often recurring, and has a poor prognosis.Surgery remains the optimal treatment for ACC, due to chemotherapy’s ineffectiveness. Our patient developed 2 liver metastases after right adrenalectomy (1.5 and 4 years). Therefore, complete surgical resection should be considered in similar cases.

“…Median OS of metastatic ACC patients varies between 10 and 20 months (2,3,5), with 5-year survival around 10% (3,6,8). However, some long-term survivors are described, including a few cases with synchronous metastases at the time of diagnosis (15,16,17,18,19,20). These observations suggest that some ACC have a slow natural history, in contrast with the usual poor prognosis of this malignancy.…”

“… 15 Surgical resection alone is often reported to be useful for the treatment of liver metastases. 16 , 17 If complete resection is possible, a long-term prognosis can be expected. Moreover, Datrice et al 16 reported that patients with ACC with recurrence >12 months after adrenalectomy had favorable outcomes.…”

“…Thus, the surgical approach remains the best-recommended treatment modality as long as the metastases of ACC are resectable completely (2). Some studies have suggested that surgical resection of the metastasis or recurrent for ACC is associated with prolonged survival in selected patients (5,12). However, a complete cure is unlikely to be achieved after metastasectomy.…”
