Repair of ruptured quadriceps tendon with Leeds–Keio ligament following revision knee surgery

Abstract: We describe the use of the Leeds-Keio ligament to reconstruct a neglected quadriceps tendon rupture following revision knee arthroplasty. The Leeds-Keio ligament has been used in the treatment of patellar tendon ruptures complicating primary knee arthroplasty with good result--but may, as this report shows, also be successfully applied to address deficiencies of the quadriceps tendon in the revision setting, with continued good function for 2 years.

“…There are reports of favourable outcomes if the tendon can be approximated following medial and lateral releases [67]. When this proves impossible, surgical options include tendon lengthening techniques such as V-Y plasty and the Codvilla technique [54,64], hamstring grafting techniques [67] and synthetic Leeds ÀKeio ligament implantation, with the added benefit of its collagen inducing properties [68].…”

“…Аутопластика при разрыве связки надколенника сухожилием m. semitendinosus приводит к неудовлетворительным функциональным результатам: максимальное количество наблюдений включало 7 пациентов, при средней амплитуде движений 80° у 5 была разгибательная контрактура [14,33]. J.w.…”

“…Patellar tendon rupture is a devastating complication, which, while rare in primary procedures, 38 becomes more common in revision cases. 39,40 A prosthesis design that leads to reduced forces in the quadriceps muscles and, by implication, in the patellar tendon will reduce the chances of such a failure. Second, lower quadriceps forces will benefit recipients of hinged TKRs, who are often elderly and have poor muscle mass, by reducing the work they have to do in activities such as rising from a chair and stair climbing.…”
