Extensor mechanism rupture is a serious event requiring prompt diagnosis and treatment. Patella fractures are reportedly six times more frequent than soft tissue injuries such as quadriceps or patella tendon ruptures. Classically quadriceps and patella tendon ruptures are seen more in males, with those over 40 predominantly suffering from quadriceps tendon ruptures, often associated with an underlying condition, whereas patella tendon ruptures are mostly associated with sport injuries and are commonly seen in the under 40s. Almost all types of extensor mechanism ruptures benefit from early management which typically involves surgery. Diagnosis can be deemed easy to make by demonstrating inability to actively extend the knee, this however can be easily overlooked and missed in a busy emergency department leading to a late diagnosis and necessitating more complex surgery. Earlier surgical intervention and rehabilitation tend to produce improved outcomes.

Несостоятельность разгибательного аппарата коленного сустава является абсолютным противопоказанием к эндопротезированию, требуя хирургической коррекции или изменения тактики лечения пациента. Цель исследования-провести ретроспетивный анализ результатов хирургического восстановления непрерывности разгибательного аппарата при первичной и ревизионной артропластике коленного сустава. Материал и методы. С 2006 по 2015 г. авторами выполнено 25 операций по восстановлению непрерывности разгибательного аппарата при первичной и ревизионной артропластике коленного сустава у 24 пациентов (20 женщин и 4 мужчин, в возрасте от 25 до 71 года), в том числе 5-при первичном и 20-при ревизионном эндопротезировании коленного сустава. Виды несостоятельности разгибательного аппарата были следующими: повреждение связки надколенника-у 15 (60,0%), перелом надколенника-у 5 (20,0%), повреждение сухожилия четырехглавой мышцы бедра-у 3 (12,0%) и прочие-у 2 (8,0%) пациентов. Использовали четыре хирургических методики: рамочную аллопластику связки надколенника с армированием проволокой-у 6 (24,0%) пациентов, аллопластику связки надколенника сухожильным трансплантатом с костными блоками-у 9 (36,0%), пересадку комплекса аллотканей разгибательного аппарата-у 7 (28,0%) и остеосинтез надколенника-у 3 (12,0%). Результаты. Непосредственные исходы операций оценены у всех пациентов, осложнений в раннем послеоперационном периоде не отмечено. Клинико-рентгенологические результаты лечения по шкалам KSS и wOMAC оценены у 18 (72%) пациентов в среднем через 44 месяца (от 6 мес. до 9 лет) после операции. Восстановление разгибательного аппарата привело к существенному снижению боли у 38,8% пациентов, стабилизации суставау 83,3%, увеличению активного разгибания голени в среднем на 10°. Заключение. Таким образом, восстановление разгибательного аппарата при эндопротезировании позволило существенно улучшить функцию коленного сустава и опороспособность нижней конечности. Тем не менее, оценка функции коленного сустава по шкалам KSS и wOMAC остается низкой, что необходимо учитывать при планировании хирургического вмешательства. The number of total knee replacements being performed worldwide is undergoing an unprecedented increase. Hinged total knee replacements, used in complex salvage and revision procedures, currently account for a small but growing proportion of prostheses implanted. Modern hinged prostheses share the same basic configuration, allowing flexion-extension and tibial rotation. One aspect on which designs differ is the anteroposterior location of the hinge. A more posterior hinge is designed to increase the patellar tendon moment arm, reducing the quadriceps force required for a given activity and benefiting the patient. Five commonly used total knee replacements were evaluated in terms of quadriceps force and patellar tendon moment arm using a laboratory-based rig. Significant differences were identified between the five prostheses in quadriceps force and patellar tendon moment arm. Analysis of the correlation between these two parameters indicates that while patellar tendon moment arm influences quadriceps force, it is not the only factor. Also important is the lever function of the patella, and it is suggested here that the non-physiological nature of the prosthetic patellofemoral geometry may result in unnatural joint function. Thus, a thorough understanding of the resulting kinematic function of hinged total knee replacements is becoming increasingly important in complex revision total knee replacement to meet rising patient expectations and functional demands.
