Background: The repair of large nasal septal perforations (NSPs) is one of the most challenging procedures in nasal surgery. The aim of this prospective clinical study was to determine the efficacy of using a pedicle inferior turbinate mucosal flap combined with temporal fascia to repair a large NSP. Method: Between January 2008 and December 2015, 17 consecutive patients with large NSPs underwent septal perforation repair via an endoscopic approach using a pedicle inferior turbinate mucosal flap combined with temporal fascia. Results: Complete closure of the perforation was achieved in all patients. Conclusion: The pedicle inferior turbinate mucosal flap combined with the temporal fascia technique can easily solve this challenging problem, and the current data from this prospective study suggest that this technique shows promising results.

Nasal obstruction (NO) is defined as the subjective perception of discomfort or difficulty in the passage of air through the nostrils. It is a common reason for consultation in primary and specialized care and may affect up to 30%-40% of the population. It affects quality of life (especially sleep) and lowers work efficiency. The aim of this document is to agree on how to treat NO, establish a methodology for evaluating and diagnosing it, and define an individualized approach to its treatment. NO can be unilateral or bilateral, intermittent or persistent and may be caused by local or systemic factors, which may be anatomical, inflammatory, neurological, hormonal, functional, environmental, or pharmacological in origin. Directed study of the medical history and physical examination are key for diagnosing the specific cause. NO may be evaluated using subjective assessment tools (visual analog scale, symptom score, standardized questionnaires) or by objective estimation (active anterior rhinomanometry, acoustic rhinometry, peak nasal inspiratory flow). Although there is little correlation between the results, they may be considered complementary and not exclusive. Assessing the impact on quality of life through questionnaires standardized according to the underlying disease is also advisable. NO is treated according to its cause. Treatment is fundamentally pharmacological (topical and/or systemic) when the etiology is inflammatory or functional. Surgery may be necessary when medical treatment fails to complement or improve medical treatment or when other therapeutic approaches are not possible. Combinations of surgical techniques and medical treatment may be necessary.

La riparazione della perforazione del setto nasale rappresenta delle più complesse procedure chirurgiche nasali. Nel corso dellultimo decennio, sono state descritte numerose tecniche endoscopiche di riparazione, con una percentuale di successo variabile fra il 76.4% e 100%. I vantaggi di questa tecnica sono la mini-invasività (nessuna cicatrice esterna), unottima esposizione del campo operatorio (con una migliore visualizzazione delle strutture anatomiche) e un buon controllo visivo dei margini della perforazione. Possibili svantaggi sono un maggior tempo operatorio ed una maggiore difficoltà di esecuzione che richiede curve di apprendimento di anni. In questa review riportiamo lesperienza di numerosi lavori pubblicati sulla riparazione endoscopica delle perforazioni del setto nasale, mettendo a confronto la percentuale di successo, il diametro della perforazione e i materiali utilizzati per la riparazione.
