Renal oncocytomatosis

Abstract: Renal oncocytoma is an uncommon benign renal neoplasm of unknown etiology. Bilateral, multicentric renal oncocytomas are rare and diffuse renal oncocytomas are even rarer. We report a patient with incidentally detected marked bilateral nephromegaly due to innumerable oncocytomas. The term "oncocytomatosis" should be strictly applied to cases in which oncocytomas diffusely infiltrate the kidneys.

“…Several papers have described cases with multiple and bilateral renal oncocytomas, but only a few appear to have been renal oncocytosis. [4][5][6][7] Due to the association of different morphologic patterns, renal oncocytosis raises the question of the possibility of a close relationship between renal oncocytoma and chromophobe renal cell carcinoma. Renal oncocytosis may also pose a diagnostic problem in surgical pathology, because of the different prognoses of oncocytoma and chromophobe renal cell carcinoma.…”

“…The pre-operative diagnosis cannot be conclusively established by the present imaging techniques [6], although a central stellate scar or the lack of necrosis may be suggestive in large lesions. Cytology [8] or a biopsy [9] could help, but some renal cell carcinomas have a high percentage of eosinophilic cells or are associated with renal oncocytomas in 10% of the cases [1].…”

“…First described in 1982 [3], oncocytomatosis represents a unique entity of a multiple diffuse bilateral oncocytoma. Today only 3 cases of bilateral oncocytomatosis have been reported [5,6].…”

“…In 1982, Warfel and Eble described renal oncocytomatosis after identifying more than 200 foci of ROs in one patient [11,12]. The term ‘oncocytomatosis’ should be strictly applied to cases in which oncocytomas infiltrate the whole kidneys, as in our case [13], and correct diagnosis is particularly problematic. Cases of metastatic tumors have been documented [6,14], but subsequent reports have identified subtypes of renal tumors with similar histological morphology but malignant potential.…”
