Renal Needle Biopsy of the Transplant Kidney: Vascular and Urologic Complications

Abstract: The authors reviewed the complications of 114 needle biopsies performed in 103 transplant kidneys. The diagnostic and therapeutic approach has changed markedly as a result of the introduction of Doppler ultrasound and the possibility of selectively embolizing fistula tracts. Macroscopic haematuria was the most frequent complication (5 %); in one case it appeared 1 week after biopsy and was complicated with obstructive anuria due to a pyeloureteric clot. The incidence of arteriovenous fistulae was fairly low (0… Show more

“…These are important criteria to decide for which period it is justified to wait for diuresis and creatinine clearance in a nonfunctioning kidney. A prolonged waiting period is associated with several disadvantages including the side effects of the immunosuppressive treatment (27), prolonged hospital admission, and complications from other investigations such as biopsies (28).…”

“…Macroscopic hematuria after biopsy occurs in 1%-7% of biopsies and is usually self-limiting (31)(32)(33)(34)(35). Color Doppler US can easily demonstrate arteriovenous fistulas (AVFs) and pseudoaneurysms as localized areas of disorganized color that extend outside the confines of the normal vessel in the renal parenchyma.…”

“…High blood pressure, systolic >170 mmHg [63] 13. Renal grafts tend to demonstrate more hematuria [101], yet less overall hemorrhage [25,29,48,63,81,[102][103][104] Follow-up strategies to reduce complications Post biopsy strategies to reduce complications 1. Post-intervention, thorough compression of the biopsy area is performed before placing the patient on a sandbag for native, or a sandbag on the kidney for transplanted kidneys, for at least 4 hrs (nevertheless there is no evidence to do so).…”
