Summary The records of fifty-five patients with secondary renal amyloidosis have been scrutinized in order to evaluate statistically the correlation of the various clinical features and biochemical alterations with the severity of histological lesions in this disease. An ‘amyloid factor’, calculated on the basis of the extent of glomerular amyloid deposition, was utilized for the grading of the latter. A significant correlation of the histological grading was observed with the duration of predisposing illness (P <0·02), alteration of total serum protein (P <0·001) and functional impairment as determined by the creatinine clearance test (P <0·002). There was, however, no significant correlation between the extent of glomerular lesions and clinical severity, the duration of oedema, the degree of proteinuria and the levels of blood urea, serum albumin and serum cholesterol.

“…Nevertheless, our findings show that the fine granular deposits in the mesangium correspond to protein precipitates which, according to Olsen, could precede the increase of basement membrane-like substance in this region. Amyloid deposits (Teilum and Lindahl, 1954;Heptinstall and Joekes, 1960;Olsen, 1972;Lendrum, Slidders, and Fraser, 1972) could neither be demonstrated by light nor by electron microscopy in our cases. Thus, although we have not completely clarified the nature of these nodules of plasmacytoma, our electron microscopic results point strongly toward a structure consisting of protein deposits and a scanty collagen fibre synthesis in the marginal sections of the glomerulus.…”

Light microscopic examination of the kidneys in 170 patients with multiple myeloma revealed three from patients without diabetes with glomerular nodules that were indistinguishable from those of diabetic glomerular sclerosis. Electron microscopic studies showed the nodules to be deposits of fine-grained material that corresponded to precipitations of protein but lacked the basement-membrane-like material characteristic of nodular glomerulosclerosis in diabetics. What is more, a unique collagen was observed in the marginal area of the nodules and in the periglomerular interstitium. The possible causes of the origin and distribution of this collagen not so far described in the human kidney are discussed.

SummaryTwo hundred and thirty-three patients with renal amyloidosis were studied in an attempt to identify the incidence and pattern of the disease in northern India. The incidence of amyloidosis was 1 01% of 6431 postmortems and 8-4 % of 1980 renal biopsies from patients who presented with clinical evidence of glomerular disease. Two hundred and three patients (87 1 %) had secondary amyloidosis, 22 (9-4 %) had primary amyloid and 8 patients (3 5 %) had amyloidosis associated with multiple myeloma. Tuberculosis of various organs was the commonest predisposing disease accounting for 59 1% of secondary amyloidosis, followed by chronic suppurative lung disease in 241 %. Rheumatoid arthritis, chronic osteomyelitis and lepromatous leprosy were seen in a small percentage of patients (2 to 8 %). Proteinuria of varying degree was present in all the 233 patients and 12-9 % of them had a daily protein excretion of more than 10 g.Post-mortem examination of 65 patients with renal amyloidosis showed that 75-3 % also had amyloid deposit in the spleen, 63 % in the liver, and 50 8 % in the adrenals.
